The Trio summon a demon to poison Buffy. This poison causes Buffy to think she is in a mental institution, where her mother and father are there. In the mental institution, the doctor tells her that everything in Sunnydale isn't real (including Dawn) and that she made them up. He also tells them to get better again she must kill her friends and sister.
Why It's So Good - Reality can be what we make it. Taken somewhat literally here. You can see the temptation for Buffy to leave her life given the trials she has faced, and the writing does a good job at convincing her (and the audience) of her absurd life.
