- Oliver has his shirt off when he is in the hideout, experiencing pain from the vertigo the count injected him with. He also has his shirt off when he wakes up.
Violence & Gore
- Oliver shoots someone with an arrow during the intro. After injecting him with vertigo, a man tells another man that he has the option to kill himself and gives him a gun. The man then shoots himself. All that is seen on-screen is the flash of the gun, and then the man's body on the floor. A pool of blood is visible beneath the man's head. Oliver strangles a man to death. Later, you find that he only temporarily killed the man, and he wakes him up, then knocks him out with a punch to the face. During a flashback, the torturer fights a man in hand-to-hand combat, then kills him with his sword. There are audible noises of breaking bone. During a flashback, Yao Fei fights Oliver hand-to-hand and lands several blows with his feet and fists. He then chokes Oliver out. Yao Fei pushes Oliver over a cliff into a river. Oliver has visible wounds on his face. Oliver fights several men in a stairwell. Oliver throws an arrow into a man's chest. Several men shoot automatic weapons at Oliver. Oliver throws a man onto and through a glass table.

- The fact that Thea took vertigo is mentioned in court and at the Queens' home. A man injects pure vertigo into another man's bloodstream, putting him into excruciating pain. When Oliver wakes up, he says he feels like he's been in the worst hangover of his life. Diggle says that coming from a guy who spent most of his 20s in a hangover, that's really saying something. Oliver injects the Count with a dose of the pure vertigo.
Frightening & Intense Scenes
- The count injects Oliver with the pure vertigo and he experiences severe pain, When Diggle gets him back to the hideout, he nearly chokes him, and then screams in pain. The Count is seen in a hospital gown restrained onto a gurney, struggling and shaking. Off-camera, he screams in pain.
