- Recuperating from trauma, Jennifer remains in danger as she returns to a life she doesn't remember.

- On a rainy night, a woman (Brenda Song) flees down the street from an unknown pursuer. The pursuer is in a truck and chases the woman inside an empty house, and then has put a winch on her car's axle to prevent from escaping. The woman runs away on foot.
She is struck by an oncoming car of a stranger, leaving her injured and unconscious. The person driving the car calls an ambulance and the woman is taken to a hospital. She has a punctured lung and blunt trauma injuries on her face. She has internal bleeding in the brain with possibility of total memory loss. Nurse Masters (Ashley Scott) is attending to the woman. A man meets Masters and tells her that he is the woman's husband. He waits overnight at the hospital to get an update from the hospital.
She wakes up in the hospital with short-term memory loss, recalling nothing about the incident or her life prior. A man by her bedside (Mike Vogel) introduces himself as Russell Williams, her husband, and says that her name is Jennifer. Jennifer is initially afraid of Russell as she does not remember if he is her husband. To help her regain memories, he shows her photos of her life, telling her that her parents died in a fire two years ago, that she had quit her job, and that she rarely talks to her friends anymore.
Detective Frank Page (Dennis Haysbert), obsessed with his work after failing to find his own daughter's kidnapper, investigates Jennifer's accident. The man who crashed into Jennifer, tells Frank that he saw a white pickup truck which was pulling a car with a winch. Frank grows suspicious of Russell, who drives a truck similar to one seen in the vicinity where Jennifer was hit. Jim Kahn (Paul Sloan) is also a witness to the accident and is shown to be visiting the hospital. Kahn leaves flowers for Jennifer at the hospital, which never reach her. Russell told Frank that the last time he saw Jennifer she went out to buy groceries and then he didn't hear from her. Jennifer's car from that night has been missing.
After Jennifer is discharged, Russell takes her to their secluded home. Jennifer is haunted by brief flashes of memories from the rainy night of her accident. She becomes uneasy by Russell's strange behavior and realizes that he has been locking her inside the house at night. She also notices that most of the photos in her home have been manipulated as some photos with Russell's face in it have inconsistencies (blonde hair on Russell and black hair in the mirror). Russell finds that Kahn is one of the witnesses who saw the license plates to the pickup truck. He follows Kahn and assaults him with a rod before killing him. Russell buries Kahn's body in the backyard of the house, where Jenniffer sees him.
Frank asks Russell to bring Jennifer in for questioning, but he doesn't. The address that Russell provided at the hospital was wrong. Frank finds that nobody called Russell on the night of Jennifer's accident, and he showed up at the hospital all by himself. Frank finds from Masters that Russell identified himself as Jennifer's husband by showing family photos and telling the doctor about a black tattoo on Jennifer's body. On the hospital CCTV footage, Frank finds that Russell drove into the hospital, on the night of the accident, on a white pick-up truck. Frank uses the tattoo to figure out Jennifer's maiden name and her address with her parents. Frank's investigation leads him to Jennifer's parents' home, where he finds their rotting corpses. He learns from Jennifer's former employer that "Russell" is actually a man named Ryan Gaerity, a temperamental worker fired over two months ago.
While Ryan is out, Jennifer finds her own ID in his wallet, which she uses to unlock his computer. She finds images of her actual husband Russell (Daniel Booko) and realizes the danger she's in. Before she can escape, Ryan knocks her unconscious and chains her to the bed while he goes out to get supplies. When he returns, the real Russell's corpse is revealed to be in his car trunk.
Frank arrives at their home and tries to rescue Jennifer, but Ryan knocks him out, locking him in a freezer. He speaks angrily to Jennifer that he had loved her for years but her lack of reciprocation and marriage to Russell enraged him. Jennifer manages to escape into the forest with Ryan giving chase. Knowing that Jennifer will never love him, Ryan decides to kill her with his gun, but Frank tackles him before he can shoot her, and Jennifer takes the gun and shoots Ryan in the arm. A wounded Ryan desperately attempts to run towards Jennifer to retrieve the gun, but Jennifer fires another shot, ultimately killing Ryan.
Three months later, Frank gives Jennifer a note from her late husband that he had found during the investigation. As she drives, leaving for San Jose, Russell's voice narrates the note, telling her how much he loves her.
