Glassman is recovering from a surgery; Tensions rise in Dr. Melendez's OR; Shaun and Morgan need to get through a surprise 36 hour shift at the ER.

In the morning after spending his night in the hospital, Dr. Shaun Murphy charmingly welcomes Dr. Aaron Glassman as he finally wakes up after the brain surgery went success.

Dr. Neil Melendez introduces a new one-time policy, made by Dr. Marcus Andrews, 36 hours non-stop shift, to improve the surgeon's performance during heavy-stress duties. Melendez split up the team. He with Dr. Alex Park and Dr. Claire Browne will be in the operation room meanwhile Shaun and Dr. Morgan Reznick volunteered in the short-staffed emergency room and both are gonna be no longer supervised directly either by Melendez or Dr. Audrey Lim.

Melendez and the team are dealing with a patient, Gina Porter (Sari Mercer) with infertility issues and pains over his stomach. In the parallel universe, Shaun and Morgan are dealing with a patient, Nate Schmidt (Amitai Marmorstein), with a more-than-4-hours erection (priapism). They give an injection as the first treatment to make 'it' calm down.

In another bed, Shaun and Morgan have to find a way to get a bulb out of a boy's mouth. Shaun has tried normal procedures but it didn't work. Morgan suggests calling Lim instead of taking a riskier treatment. Morgan has no idea what is Lim going through at that time. In the middle of arguments with the judge at a traffic court, Lim's phone is ringing which disappointing the judge. The Lim's verdict gets postponed.

Claire, who apparently disagrees with the one and half-day shift challenges the policy to Andrews but Andrews stays stick with the policy. Claire's aggravation keeps continuing during surgery. Melendez's team finds lesions over ovary and another organ of their patient which relevantly explains her very-pain feeling and infertility. After asking Claire to tell her husband that the surgery will be longer than planned, Melendez accidentally sparks irritations among nurses by saying Claire seems like having a PMS.

Shaun and Claire in a short conversation about Shaun's reluctance to talk to Lea. She suggests Shaun not to wall his self off to deal with an uneasy relationship. Shaun then found an idea to treat the bulb in the boy's mouth by wall the bulb off with a surgical pouch so the bulb can be bitten without worrying the broken piece entered the boy's throat. And it works.

Claire backs to the operation room but the nurse Ann Flores (Liza Lapira) thinks Melendez's response is unwise. They have a more intense bicker now. Meanwhile, Glassman is starting to experience post-surgery trauma.

Lim growled for waiting for a long time for her cases to be decided. She interrupts the on-going process and asking the judge to continue her case. The judge immediately decides to fine thousands of dollars and suspend Lim's driving license. Lim's upset and arrested for launch offending words to the judge.

Nate's erection comes back, both Shaun and Morgan now must find solutions without calling Lim. Shaun prefers to ask Glassman and he suggests Shaun Winters Procedure.

Melendez's team has been tiredly working in the OR to repair every single lesion in Gina's body. The surgery worsened as the patient having internal bleeding. Now they have to ask the husband, Daniel Porter (Arjay Smith) for a possibility to sacrifice a piece of an organ to save the bleeding. Melendez asks Park to meet the husband and asks another nurse to keep an eye on the patient while he wants to talk with Claire and nurse Ann over the recent communication problem among them. They blame each other and Andrews come to end the commotion.

Lim is freed from the jail, the case closed and having a little romantic night with a guy in the court until Shaun calling. He was found that it seems there's an issue in Nate's backbone near the pelvic and the MRI proof it's true. Lim comes and leads the surgery. There's a conversation about Lea. Lim suggests Shaun talk to Lea to figure out how actually she treats Shaun.

Melendez's team got the patient's husband consent, but then, they found polyps on the uterus which means the have to remove the uterus completely and ended the patient's dream to be a mother. Another solution to save the uterus has a death risk. Claire decides to save life after Gina's husband let Claire make a decision.

Both surgeries went success. Nate is happy to hear "he" is fully-functioning again and Gina is okay with the remove. Uneasy relations among Melendez's team now ended after Andrew give the final hearing. But not with Shaun, Lea has been waiting in frown outside the hospital. Shaun admits her mistakes by demanding her to go away days ago. Lea angrily expresses her disappointment to Shaun saying he is only thinking about his feeling and at the same time ignoring her feeling.

Glassman experiencing a sleeping issue since recovered from the surgery. He wakes up again and starts seeing a hallucination of her daughter.
