- One woman attempts to seduce another woman's husband while wearing a partially unzipped wetsuit. Her bra deliberately shows off a fair amount cleavage. Before the husband can finish "assisting" the woman with zipping up her wetsuit, his own wife interrupts him.
- A woman teases another woman about hiding an attractive body underneath her camp clothes. Feeling abandoned by her husband the woman decides to go ahead and remove her camp clothes. She only starts out removing her shirt to reveal a bra and cleavage.

- 6-Headed Shark Attack meets Syfy Channel's typical monster movie violence & gore factor including instances where shark victims are gruesomely killed. Almost every victim is killed in bloody fashion including being torn apart by multiple shark heads.
- A man and woman on surfboards are both killed and torn apart by the 6-Headed Shark.
- A man finds the remains of a bloody foot inside a boot floating on a sea current.
- A woman swimming out towards a raft first swims into a patch of what appears to be blood on the water. It is revealed to be the remains of a human body in red swim trunks, missing several body parts.
- The 6-Headed Shark attacks a man on a beach and rips his torso in half, causing a brief explosion of blood that lands on another man standing nearby.
- A ship is sucked into a maelstrom created by the 6-Headed Shark. A man onboard the boat abandons ship at the last minute and is shown swirling around in the vortex. He is quickly eaten by one of the shark's heads.
- The 6-Headed Shark smashes through the hull of a boat. It is attacked by a man using a makeshift spear to stab it. Stab wounds are visible on one of the shark's head.
- There are several instances when the 6-Headed Shark sustains injuries to one of its heads. Most times the monster bites off one of its own heads when injured. At least one time it bites off one of its heads to use as flying projectile.
- A man is killed inside a lighthouse when a flying shark head lands on the structure causing the top of the lighthouse to explode.

- During the movie's climax the 6-Headed Shark stalks two survivors on a rocky beach. A man fires a flare gun at the 6-Headed shark's tail that snared a net containing several gas tanks. The tanks explode, destroying all six heads. The shark's body is covered in mass amounts of blood and gore.
