Some innuendo jokes, For example, a skeleton teaches you a ledge jumping move which he calls "The pull-out Technique," When Ash doesn't react to this, the skeleton then states how he imagined the name would get more of a laugh before requesting to be left alone.

Ash explodes black chunks on death and leaves behind a ghost so the player can see where they died. Not graphic. Apart from that, there isn't really anything violent.

As with all Edmund McMillen games, the Art style is really cutesy and innocent looking, but further inspection reveals the games Dark themes and Black Comedy.
The entire game takes place in a post apocalyptic setting where Ash is one of the only survivors. Because of this, the game constantly has a morbid, depressing tone in it with themes of loneliness and despair the whole way through. Ash will meet up with various Dead, Dying or Psychologicaly insane characters who interact with Ash, most of which just want to be left to suffer in loneliness.
It's heavily implied Ash has also been driven to insanity from loneliness.
The games levels are very intense with easy to understand controls but absolutely rage-inducing difficult level design that'll push even the best platformer players to there very limits, frequently pitting players in many Do-or-Die situations.
