It is 1922. In the beginning of the episode we see the beaten up Tom Shelby lying in a HALO vest. This is a device to stabilize the neck, usually for fractures, and the first use of it was not described until 1959. A few scenes later he is sitting on the bedside without the HALO vest but with a fresh scalp wound. Although not specifically stated it would be reasonable to assume that the skull operation took place on the same day as the admission to hospital and that the HALO vest was applied directly thereafter. A HALO vest is usually worn from 6 weeks to three months and certainly never for just a few days.
At approximately 17:42, Tommy looks down to light a cigarette, and just before he looks up the frame jumps, showing that the camera has been moved slightly between takes.
