San Antonio Memorial's administrator Michael Ragosa, whose ever-worsening eyesight wrecked his surgery studies, must run a rigid budget regime to avoid his money-draining ER being closed down by the board. Dr. Jordan Alexander gets a chance as new chief of a staff consisting mainly of war veterans, like her. Brilliant surgeon TC Callahan, who was fired from the Afghan war zone for insubordination and sleeps drunk in a friendly deputy's jail, seems most unlikely to bend for the pair of them. Ragosa, whom TC first decks for opposing excessive pro-bono treatments, wins TC's respect by instructing a novel skull surgery procedure for pre-teen Matthew, whose head was nearly severed in a crash while his devoted brother Tommy drove.
