- Miles Upshur is a freelance journalist, he receives a tip from an anonymous whistleblower working at Mount Massive Asylum in Leadville, Colorado. Mount Massive is owned the Murkoff Corporation, a notoriously unethical company. The tip describes human experimentation being conducted on the patients, made more credible by the asylum's history, it was closed in 1971 in wake of a government scandal and reopened by Murkoff in 2009. Miles' cell phone reception is cut off upon arrival, like it's being jammed.
The investigation goes south almost immediately upon Miles' arrival, many rooms are the site of massacres, patients, staff, even Murkoff's private forces have being killed violently. A muscular inmate attacks Miles' and throws him through a window, Miles' regains consciousness long enough for a patient turned priest to examine him and declare him his apostle. Miles' carries on, few inmates are alive, some are docile and unaware while others are extremely hostile. From documents found Miles discovers the muscular inmate is Chris Walker, former military police, suffering from PTSD, he's ranting about containing a breach, Miles hides from him whenever possible.
After reactivating the security feed, the priest jumps Miles and sedates him, before Miles loses consciousness again, the priest shows Miles camera footage of Murkoff forces being slaughtered by an unseen force. Miles wakes up in the priest's cell, he is Father Martin Archimbaud, a patient with schizoaffective disorder, Miles is released from the cell and wanders the prison where cannibal twins pursue him, someone rescues him with a dumbwaiter, unfortunately, that someone is Dr Richard Trager, a Murkoff executive driven mad with all the other patients. Trager mutilates other patients, removing their fingers, tongue and genitals. Trager severs Miles right index finger and left ring finger and leaves him, Miles breaks from his restraints and witnesses Trager execute another maddened Murkoff executive.
Miles flees from Trager, ending when Trager is caught between an elevator and floors and crushed to death. Miles ventures on following Father Martin's guidance, he and his congregation are worshiping the unseen force that is killing the occupants of Mount Massive, it is known as "Walrider."
Walrider was a project spearheaded by a supposedly deceased doctor, Rudolph Gustav Wernicke, a former Nazi scientist recruited into the United States in 1949. Walrider was an attempt to channel nanomachines into a single entity, accomplished through dream therapy on trauma patients.
Miles makes it to asylum chapel where Father Martin has prepared a key to the lower levels of Mount Massive, before commanding his flock to incinerate him on a cross. Miles enters the lowest levels where Murkoff run their human experimentation project, along with their only successful test subject, William Hope, Walker attacks Miles again only for Walrider to slaughter Walker. Miles meets Dr Wernicke, he explains the research and his relation to Hope, the latter has grown attached the doctor. Wernicke explains that Hope's life support must be deactivated to end him and the project.
Miles destroys the systems and euthanizes Hope, however the Walrider takes possession of Miles instead. Miles hobbles back through the labs where Murkoff troops are waiting to attack with Wernicke. Miles gets back up under full possession of Walrider and slaughters the troops.
