Women Npcs will some times have bikini like armour.
If you use mods (optional) you might get nudity.
The character customization female characters wear a bra and underwear, however it is not graphic or emphasized.
The only reason there would be any kind of midrift shown on a few female NPC's is because the same kind of male NPC's don't have a shirt; It is put there to cover up, not to show anything.

Dismemberment can happen however it is rare and it also can be turned off using the settings.
Contains blood but can be turned off.
Generally a very graphic and violent game, with realistic bloody wounds, but violence can be turned off in the settings.

No blood or gore on, 10+. Blood on minimum, 11-13+. Blood and gore on minimum, 13+ Blood and gore around medium-doneness, 15+ full blood and gorce turned all the way up.
