- Talking to her friends, Lena Lawrence describes how to perform the proper ritual, the summoner has to go into a dark room, light a candle, stand in front of a mirror, and says the name "Bloody Mary" three times. Then Bloody Mary sees into the summoner's soul, and if Mary doesn't like what she sees, she scratches the summoner's eyes out.
Lena and three of her friends are in a bathroom at a house, talking about Bloody Mary. Lena explains that Bloody Mary can see the summoner's future, and tells them how to get what they want. The other girls, Maggie, Elise, and Bianca, joke about Mary helping them get boyfriends. Bianca complains about how her friends never talk about anything real, and Lena insists that Mary is real.
The girls start to perform the ritual, and Bianca panics and turns on the lights before they can finish, and goes out into the neighboring bedroom. She says that it's not funny and walks out, Elise goes out and talks to Bianca, and complains that Bianca embarrassed her in front of her friends, and Bianca doesn't have any friends so she hangs around Elise and her friends. Bianca finally gives in and says that she'll do the ritual, and Elise calls the other two girls out and says that they're going to do it in the bathroom.. They all go into separate rooms to perform the ritual, and the flame on Maggie's candle goes out. Mary appears to Maggie, and says that if Maggie obeys obey her, she'll have what she wishes.
Maggie screams, and her friends run in to see Mary scratching Maggie's face bloody. When they turn on the lights, Maggie is back to normal. Bianca runs out and they go out and tell Bianca that it's fine. Maggie says that nothing happened, Lena tells her that they heard her scream. Maggie insists that she didn't scream, and the girls admit that Mary did to appear to them and get their wishes. Bianca told her to accuse her guidance counselor of raping and molesting her, and she'd get a full ride to Yale. Mary told Lena to drop her rival from the top of the pyramid, and she'd be paralyzed and Lena would become the cheer captain. For Maggie to be with River, the boy she wants, Maggie should steal his current girlfriend's Kristen's nudes and put them on the Internet, humiliating her. Elise refuses to say what Mary told her when she said that she wants to be rich.
They figure that Mary was trying to scare them, and they admit that it worked. Maggie insists it was a mirror hallucination, but Elise doesn't believe they all had the same hallucination. Bianca doesn't believe it either, and figures that Mary is coming for them. Elise says that whatever they thought they saw, it's over and they should go home, and leaves with Bianca.
The girls return home and find their mother Robin on the couch with a man. Bianca says that something happened and that's why they're home, and she angrily tells them to get out of her house so she can have a night alone. The two girls go out to their family car to sleep, and Bianca says the hardest part is that she really wants to go to Yale. Elise tells her that she's smart enough to get into Yale on her own, but Bianca doesn't believe it. They both go to sleep, while Mary watches them from the rearview mirror's reflection.
The next day at school, the cheer captain Anna complains that Lena didn't dress up for school spirit week. Lena says that dropping Anna might not be a bad idea. In class, the teacher Ms. Brooks says that they're going to spend the rest of the quarter talking about the laws of reflection and refraction. She says that reflections are as real as they are, and most indigenous cultures believed that reflections were portals to another world. She flips over her whiteboard to reveal a mirror on the other side, Elise and Maggie see the mirror crack and Mary appear in the mirror, and she tells them to fulfill their pacts with her. Maggie screams and runs out, and Lena goes after her.
They meet Elise and Bianca under the bleachers, and Maggie tells the others what happened. Lena figures they have to do what Mary said, and talks about how her mother wasn't invited to a slumber party when she was a girl, and the girls at the party died or ended in a psych ward, one by one. She didn't think it was real so didn't mention it to the others. Bianca worries that if she accuses her councilor Mr. Miller of rape, it will ruin his life, but if she doesn't she's going to die. Elise says she'll figure out a way to keep Mary from killing them,
Later, Bianca meets with Mr. Miller, who goes over her essay for Yale and finally tells Bianca that they need to feel her desperation. Miller tells the girl that she has to use her advantages, and takes her hand. A nearby mirror in the office cracks to Bianca, and an image of Miller hanging himself appears in it. She quickly leaves.
Elise is in the library researching the legends about Mary. She finally finds a reference to Mary when she was a human, Margaret Worth, when she was a slavecatcher and lived in an old cabin in the woods.
Maggie is at her locker when River gets into his nearby and casually greets her. As she starts to say that breaking up with him was a mistake, Kristen comes over and kisses him. Mary appears in a mirror in Maggie's locker, and Mary gasps in shock. River asks if she's okay, and Maggie says that she is and the couple walks off.
Lena is in the locker room putting on her cheer outfit, and Anna complains that she's late. Lena looks into the mirror and sees an image of Anna's neck breaking. She meets with Bianca and Maggie and says that Mary is appearing everywhere. Bianca tells the others that they should give Elise a little more time. Lena refuses, says that have to do what Mary says rather than die, and leaves.
Bianca finds Elise in the library and says they have to stop Lena before she paralyzes Anna. The two sisters go to the gym just as Lena, Anna, and the others perform their stunt routine. They join Maggie in the bleachers and watch, and Elise yells at Lena that she's got it. Lena helps catch Anna, then runs off saying that she can't.
That night at their house, Elise covers the mirror and figures that Mary is both spiritual and physical. Bianca says that it isn't Elise's fault, and Bianca chose to say the words so she has to say the price. She figures it would be easier to do what Mary says, but Elise says that she convinced Bianca to do it so she's to blame, and she's going to keep Bianca innocent as long as she can. Bianca asks Elise to say, but Elise says that she has to go to work to provide food for their family.
Bianca goes to bed, and the cover over the mirror falls off and Mary appears. She wants Bianca to complete the task she set for the girl by midnight the next day. Later, Elise comes home with food and finds Bianca on the doorstep, crying. Bianca says that Mary gave her a deadline. Maggie runs up and says that Lena is dead, her eyes clawed out and her body left in the bathtub. The cops think some homeless man followed Lena home and tries to rob her, and Bianca figures that Mary is coming for her next. Elise tells them to not give up, and to not do anything stupid. They go to Bianca's room and Elise breaks the mirror.
The next day, Elise goes to the local museum and asks the curator, Robert, if he any information about Worth. He says Worth wasn't a witch despite the accusations against her, and shows Elise and explains that Worth used a former slave that was no in her employ to lure escaping slaves in. Worth lured them into the cellar, locked the door, and left them to die of dehydration and hunger. Everyone thought Worth was a witch because of the inhumane ways she killed people, but Robert tells Elise that Worth wasn't a witch. Eventually someone stabbed her dead and left her body in the cabin while opening the cellar door, but the killer was never found. Robert shows Elise the silver dagger that they believe was used to kill Worth, and the dagger is Caribbean in origin. As Elise leaves, she sees where Robert puts the keys to the case where the dagger is kept.
At the school, Bianca asks Maggie if she wants to come with her to see Lena's parents. Maggie is downloading Kristen's nudes, and Bianca tells her not to do it and give Elise the time she asked for. Maggie figures that it's her or Kristen, and finds a sex tape from Kristen with Kristen with another boy that isn't River. Maggie figures that Kristen brought it on herself, and Bianca asks Maggie how she could be happy with River if Kristen kills herself because the tape is leaked. Maggie cries and says that she doesn't want to end up like Lena, and Bianca walks off. Reconsidering, Maggie doesn't post the tape.
At home, Elise goes over the papers and photos she's assembled. Robin comes by and wonders why Elise is there, and says that she doesn't waste time wishing that she never had her daughter when Elise asks. Elise finds Bianca in the kitchen and tells her sister to come with her, and they're going to commit grand larceny and then cut someone.
At the museum, Bianca convinces Robert to let her in to finish a paper. Once he lets her Bianca in, she distracts Robert while Elise steals the keys and then the dagger. Once Elise has the dagger, Bianca cuts off Robert's talk and leaves, and then they go to Maggie's house so she can take them with her and Elise. They go around the back and find an unlocked door, and find Maggie dead. Elise tells Bianca that there's only one way to stop Mary for good, and they go to the Worth cabin in the woods.
The sisters go to the cabin on foot. They get lost and night falls, but there are mirrors hanging from trees and Elise figures that they're getting close. Something moves in the brush and Elise dismisses it as the wind, and they find the cabin, but there's no sign of Mary. Elise figures they have to call Mary, and the sisters approach one of the mirrors, light a candle that Elise brought along, and Elise says Mary's name three times. Mary asks if Elise brought it all. Bianca has no idea what Mary is talking about, and Elise says that she brought it all.
The night of the party, Mary told Elise that she really wants security and power.
In the present, Mary explains that she needed her dagger and the blood of three innocents. Elise tells Bianca that she didn't want to, but didn't have a choice. Bianca realizes that her sister killed Lena and Maggie, and that Bianca is the third innocent. Elise asks Bianca to forgive her, and Bianca runs away. Her sister chases her with the dagger, while Mary follows in the mirrors. Bianca hides, and Elise yells that Mary realized that Elise truly wanted never wanted to struggle again. All she has to do it set Mary free to gain unlimited power. Elise then comes up behind Bianca and knocks her out.
Elise takes Bianca back to the outside of the cabin and prepares to sacrifice her sister. She says that she has to do it, Bianca grabs Elise's hand as Elise stabs downward, and Bianca manages to grab the dagger and drive it into Elise's eye. Mary tells Bianca that all she does is reflect truth, not kill, and Elise let her desperation turn her into a killer. She admits that she hasn't been innocent in 150 years, and she did kill once when she was mortal. Because of that, Mary is imprisoned in the mirrors.
In the past, Mary comes down into the cellar and tells the escaped slaves that her people left her there years ago with Worth, who has taken care of her ever since. The head slave asks Mary for pity, saying that she and the others will be Mary's family. Worth comes down the stairs and says that the slave catchers are there, and the men tie up the former slaves, separate them, and drag them away. Mary looked into a mirror and remembers who she is, a servant of the most high Mami Wata. She calls on the goddess to give her justice, then kills the slavers with her magical powers and takes vengeance against Worth, stabbing her with the dagger, then instinctively killed the leader when she tried to intervene. Mary sullied her soul before the goddess. Mama Wata found a place where Mary could live with her truth, in the mirror.
In the present, Mary says that now all she sees is truth. She tells Bianca that she sees innocence in her, and she killed Elise that she had to because she had to survive, but Bianca has a pure heart. Bianca, crying, says that her two friends and Elise are dead, and she has nothing. Mary says that they have both been parted from the ones they love, and Bianca says that the suffering has to end. She picks up the dagger, cuts her palm and puts her blood in the third jar, and holds it up to the mirror. Mary takes the jar and drinks the blood, and says that Bianca set her free.
Mary reaches out and takes Bianca's hand, and takes her place, leaving Bianca trapped in the mirror. As Bianca pounds on the glass, Mary says that she needed to be purified by innocence and Bianca will be the new guardian of truth, the new Bloody Mary. Mary tells Bianca to do what she will with the ages of the truth that will be opened to Bianca, and walks away. Trapped in the mirror realm, Bianca hears voices chanting "Bloody Mary".
In her bathroom, a girl lights a candle and performs the ritual. Bianca appears to the girl in her mirror and the girl screams.
