A boy runs out of a house and it closes behind him. He listens as something bangs on the inside of the door. The door swings open, and...
The boy, now an adult, Axel Hernandez, looks at the door. The housekeeper, Tia Flora, opens the door and welcomes Axel. She says that Axel's father Rodrigo doesn't have much time, and is glad that Axel came to see him and Rodrigo will finally get some rest. Axel hopes that Rodrigo can help him with getting some rest, and Tia tells him that his brothers are inside with everyone else.
Inside, the family has gathered and stares at Axel as he comes in. Axel goes to his unconscious father's bedside and looks at a photo of his mother Sonya. Rodrigo wakes up and admits that he punished Axel for his wife's death, but he didn't mean for what happened later. Axel says that since he got sober, he started to remember things. He read his father's biography, and started to remember things. Rodrigo mutters about "El Coco" and the lights go out.
Axel runs out and finds his father's two older brothers there. He says he's six months' sober, and always looked away from what Rodrigo dead. The younger of the two brothers, Javi, says that he believes Axel but they shouldn't talk about things in front of the others. The older of the two, Dante, doesn't believe Axel and walks off. Javi tells Axel that they'll talk later, and Axel goes to the bathroom and looks at himself in the mirror. He runs hot water, and the mirror steams up. Axel wipes the steam away, and remembers the same thing happening, and some creature scratching his hand. Snapping back to the present on the floor, Axel finds similar scratches on his hand.
Going out to his car, Axel finds Dante and Javi waiting for him. Dante says that running away is what Axel does best, and Axel tells his brothers that he saw El Coco again and it scratched his head. Dante accuses Axel of hurting himself to get attention, and Javi tries to smooth it over. Axel accuses Javi of doing the same thing he did when they were younger, and figures that Rodrigo's temper tantrums and El Coco are somehow connected. Dante walks off, disgusted, and Javi doesn't believe Axel. Axel says that he knows what he saw, and Javi asks him to come back inside.
Axel does so and finds a library with Rodrigo's books about El Coco on the shelves. Tia comes in talking to two of the guests members, talking about how much Rodrigo did for his family. The guests leave, and Tia says that they have to go home and they should stay overnight to watch Rodrigo
That night, the three brothers wait in the living room. They hear banging and Javi offers to go check it out. He goes out to the garage where the noise came from, and finds a rocking chair rocking back and forth, and something in the shadows behind him. The lights flicker and some creature slashes at Javi. He falls down, and Axel comes out. Javi claims that he just tripped, and tells Axel not to think about dark stuff too much. Axel accuses his brother of avoiding facing the truth, but Javi says that he isn't and leaves... while a nearby cardboard box has claw marks on it.
The three brothers go to the kitchen to have a snack, and Axel insists that El Coco was in the garage and Javi saw it. Dante tells Axel to let go of his fantasy, and says that he had it good as a kid because their father's books supported them. Axel tells Dante that he has no idea what he went through, and his older brothers left when he was a kid. He had to deal with Rodrigo and his rage, and Dante insists that wasn't the father he knew. Axel tells Dante that when Rodrigo got angry, his demons would come out, and figures that El Coco is the demon. He says that whenever Rodrigo got angry, El Coco was there, and shows Dante one of their father's books about El Coco.
Dante dismisses it as another of Axel's drug-induced delusions. Axel insists that he's clean, and Dante goes to check on the unconscious Rodrigo. Dante hears something in the walk-in closet, and goes to check it out. The doors close behind him, and something with red eyes lunges at him. Axel and Javi come in response to Dante's screams, open the doors, and find Dante on the floor, clutching at his scratched arm.
The brothers go outside and Dante tells Axel to let it go. Axel points out a photo of El Coco in one of the books, and reads the part about how the blood doesn't feed El Coco, but the boy's fear that would one day turn to rage. Axel points out a scar on his cheek and reminds Javi of the day he got it, but somehow it's in Rodrigo's book. He figures that Rodrigo uses him in his books, and says that Dante would have trouble reconciling his image of Rodrigo with what Axel is saying, in return for the money Rodrigo gave him.
Tia comes out and Axel tells her that El Coco is a demon that Rodrigo summoned when he drank and got angry. The housekeeper dismisses El Coco as a fairy tale she and Rodrigo told the brothers as children, and is part of their culture. She sways that she forgot her phone in the house, and Dante and Javi to get it. Once they're alone, Tia tells Axel to let it go for the good of the family.
Inside in the bathroom, Axel looks at the pills. The lights go out and remembers when he was a boy, hearing El Coco. He dumps the pills in the sink and hears the monitors sound an alarm. Axel goes to confront Rodrigo, and asks how he summoned El Coco. Rodrigo directs him to his desk, and the lights flicker as Rodrigo flatlines. Something growls from the shadows, and a figure appears behind the curtains. Axel runs out into the hallway, and the door slams behind him as his brothers yell.
Axel goes to Rodrigo's desk and finds a sketch of El Coco towering over a boy. There's an entry by Rodrigo saying he inherited El Coco from his father, and hopes his sons won't inherit his. Axel goes back and finds El Coco towering over his brothers, and says that it wasn't Rodrigo that summoned the demon, but him. As El Coco approaches him, Axel says that he can't undo the past, and says that he forgives all of them. The lights flicker and El Coco howls... and then disappears.
The next day, the three brothers sit outside and Dante says that he should have believed him. Javi tells Axel that he should have been there for him, and Axel tells them that now that they believe him is what matters. He warns his brothers that El Coco never really goes but lingers in the shadows, feeding on their rage. As they embrace, Tia comes out and says that she knows Rodrigo is dead. The brother says that Rodrigo was no angel, and Tia knew what was going on in the house the entire time. She turned her back on Axel whenever Rodrigo got angry, and Tia tells them not to talk about such things Tia tells Axel that he needs to think about the family, and the brothers walk away from her in disgust.
Tia goes through Rodrigo's things, putting away his books. She opens one of them, crying, and then throws it at a nearby mirror in anger. El Coco appears in the reflection.
My family has a secret... It has long black claws and glowing red eyes.
