- Five convicts escape from a maximum security penitentiary. While most of them are caught quickly, the most dangerous one gets away. He is Memmo Fierro, the Mala-Noche gangbanger who killed Delko's sister and Horatio Caine's wife.
Soon, he appears on a private boat. The owner bravely shoves his girlfriend into a closet to save her while confronting Memmo. Unfortunately, he can't get to his gun in time. Instead, Memmo gets it, takes the man up on deck, tortures him, ties his feet to the anchor and drops it, drowning the man. He then steals jewelry and leaves.
Horatio questions Ivonne Hernandez, Memmo's former lover and mother to his daughter, Elsa. She lies at first about Memmo, then comes clean when Boa Vista sees she is wearing jewelry from the boat robbery. Ivonne says Memmo came to see her, but lies again about knowing where he might be going. She doesn't want to jeopardize her chances of getting Elsa back from the foster care system.
Cut to a hospital emergency room. Memmo questions nurses about when they started working at the hospital. When the first two mention starting the previous summer, he guns them down in cold blood. He is about to kill a third when a Miami Dade police officer strangely fires just one shot from his gun and then stands around for Memmo to shoot him down, allowing the killer to take his weapon and a police radio.
Later, H gets a call from a case worker for the private company handling Miami's child care system. Memmo has him at gunpoint and kills him.
While investigating the case worker's killing, H learns the name of the foster mother and he and Wolfe head to the address. They are too late. The foster mother is horribly wounded but reveals that Elsa has been missing for two days.
More investigation shows that Ivonne broke into her daughter's room. When Natalia questions her, she admits she told Memmo about the ER nurses who turned her daughter away the previous summer when Elsa came in with second-degree burns from a household accident. Memmo was getting revenge on the nurses, based on Ivonne's misinterpretation of the events (she failed to mention that the ER was very busy and Elsa could not be seen immediately; the nurses, though somewhat rude, suggested Elsa, her foster mother and the case worker head to another clinic for help).
Ivonne also reveals she told Memmo the case worker's identity. Of course, it is too late.
However, it seems something's hinky (according to Boa Vista) with the foster care company. H finds out that the company manager McGuire has Elsa and, using GPS, corners him outside a gated community where we learn McGuire was going to "sell" Elsa to a rich family. Since Memmo still has his police radio, he arrives in a stolen cab and takes McGuire hostage.
Memmo tells Elsa he loves her and then drives off with his hostage.
Later, H helps Elsa reunite with her mom. Meanwhile, Frank and the MD police find the cab with McGuire in it, a bullet to the temple. Memmo is shown calmly walking away.
