A Paramedic put a kid on ECMO?
andrewd-815 January 2022
Warning: Spoilers
In this episode a paramedic put a child on ECMO. Puting anyone on ECMO is a highly specialised procedure performed by a physician, in all circumstances. Usually by a cardiac surgeon, there is no place in the world where a paramedic would put anyone on ECMO. The child also recovered from his hypothermic cardiac arrest within 30 seconds of being on ECMO, that does not happen.

When the paramedic put the child on ECMO they were not sterile, causing the child to have a life threatening infection. The line that they inserted on camera was a small central line, ECMO cannulas are the size of a garden hose. They were rewarming him on ECMO, the warmer is a separate, large, piece of equipment that was not present in the back of this van. Without this warmer the ECMO circuit will only warm a person to room temperature, as seen in the scene in the van it is snowing. It takes several hours to rewarm a hypothermic patient, if you go too fast you will kill them.

I know what I am talking about, I take care of ECMO patients daily. This child has a 30% chance of survival if ECMO was appropriately initiated.

Believable?
Bowler30011 January 2022
Warning: Spoilers
I think this show was a little farfetched. Anyone else have views on this? I cannot believe that that little boy lived after all that time underwater. Plus the fireman that saved the little boy was seen unclothed and laying in the snow and is revived and lives. Seems to Hollywood.

Less Fantasy and more drama, please
spikeluvr20 January 2022
The show has becomes more science fiction and fantasy than drama with this ice storm storyline. Some parts were very good and typical for the show. Especially the collapsed shelter and the boy under the ice. The rest of it was ridiculous. The idea that a firefighter would deliberately leave a warm house in anger and drive off into that ice storm shows incompetence and arrogance. The captain walking out into a whiteout chasing after someone who fled his house is absurd rather than heroic. The whole illegals plot line doesn't make them at all sympathetic although it's meant to. The writers need to dial back the fantasy and the political messaging and get back to the drama they did so well last season.

What happened to the screenwriters?
athirn14 January 2022
So many dumb things in this episode, so I can't even say what was at least okay. There's no sense in key events, as if the screenplay's been written by a child who just doesn't know life well enough to make up a more realistic grown-up story. Come on! Don't turn this into a teenagers fantasy drama, please. 9-1-1 is about responsiblity and professionalism, remember? And no farfetched overblown coincidences.

Might have been inspired by real event

Warning: Spoilers
In Austria a three year old girl fell in an icy pond, she was underwater for a while. Rescue workers did arrive and started procedures. She was put on a ECMO at the hospital after other procedures were done. She was comotose, but recovered.

That had taken a lot of effort in a hospital, not in the back of car.

I grew up in snow. That was not accurate for how that worked.

You don't walk aimlessly in the snow. As a firefighter Owen should have known better than that.

Question, if the girl was in charge of the checklist. How did they know she was gone? Wouldn't she have had the checklist on her?
