- In 1981 the Cold War is at its height, three CIA operatives, Russell Adler, Alex Mason and Frank Woods are in Amsterdam looking for an Iranian terrorist Qasim Javadi for his role in the Iran hostage crisis of '79, Qasim is roughened for the location of the leader of the crisis, Arash Kadavir. Kadivar is at Trabzon Airport in Turkey, the three operatives observe Kadivar arrive at the airfield in a jeep with two others in the vehicle then shoot them both. Mason makes a sniper shot that misses, they make for the airstrip as Kadivar tries to escape on a plane which is destroyed, Kadivar is dragged from the wreck, he confesses the crisis was the idea of someone known as Perseus, Adler knows that name, he then shoots Kadivar dead and orders a sweep for survivors.
Adler, Mason and Woods attend a meeting with Jason Hudson, Perseus is the code name of a Soviet spy, an elusive figure, President Ronald Reagan authorizes a black operation to neutralize Perseus. A team is assembled, comprised of C.I.A. operatives Adler, Mason, Woods and Lawrence Sims, MI6 operative Helen Park, Mossad operative Eleazar "Lazar" Azoulay and newcomer known only as "Bell," a former acquaintance of Adler in Vietnam.
The team group at a safehouse in West Berlin, in order to find Perseus they reexamine an old mission from the Vietnam War, Operation: Fracture Jaw in Da Nang, 1968. Adler, Sims and Bell are called to an American FOB to collect a valuable asset, during the fight they are attacked flying over A Shau Village, Bell and Sims assault the village and kill two Soviet soldiers in a safehouse taking their intel. They return to their objective, the FOB is under attack from Viet Cong, the assault is repelled and the American personnel hand over a nuclear warhead to the three soldiers, their helicopter is downed almost immediately, they fend yet more assailants long enough for friendly aircraft to napalm the Viet Cong.
The Black Ops team have reviewed the old intel for names of Soviet targets, one name is flagged, Anton Volkov, Russian arms dealer in East Berlin, associated with Perseus. After East Berlin, the team discover Perseus is in possession of a stolen nuclear warhead, acting on decrypted co-ordinates the team investigate a large Soviet base in the Ukrainian woods. Woods and Bell infiltrate to find it's part training ground for an invasion of America. They uncover a computer with a reference to 'Operation: Greenlight' which reveals the warhead is American and Hudson is part of Operation: Greenlight.
The team are extracted and confront Hudson, President Eisenhower concocted Greenlight in 1958, a nuclear warhead was placed under every major European city in top secret, they would be detonated in the event of Soviet invasion of Europe, in 1974 the warheads were upgraded to high-yield neutron to preserve city infrastructure. If Perseus were to detonate the nuke, the world would know it was an American make and every nation would turn against the U.S.
Woods and Mason head to Yamantau to seize an old mainframe containing the details of Soviet sleeper agents planted in the U.S during Mason's brainwashing in the 60's, they flew in with Russian double Dimitri Belikov who winches the mainframe from the base. Unfortunately, Perseus deleted all the names and the only place they're kept in now is in the Lubyanka building in Moscow. The black ops team breaks in with the help of Belikov and infiltrate the underground bunker, copying the names to disk.
Once back at the safehouse the disk is analyzed for sleeper agent names, one name, Theodore Hastings, is a nuclear engineer in Salt Lake City, associated with Greenlight. A surveillance team tracks him to a vacant facility in Cuba, it's believed the stolen nuke is there. The team infiltrate the site, fighting through the Cuban Army. Bell checks the camera feed for Hasting and witnesses Perseus execute the scientists including Hastings, Perseus had him reverse engineer the detonation codes granting him access to all Greenlight bombs.
Bell, Park and Lazar head for a roof for extraction via sky-hook, an RPG injures them and Bell has a choice to save one or neither. An injured Bell is taken the safehouse, Adler begins hypnotherapy on Bell regarding the Vietnam conflict, Bell once met Perseus in person in a bunker. Bell attend a meeting where Perseus discussed to fellow spies his plan to hijack Greenlight and turn the world against the U.S and toward the Soviet Union, when the time was right the bombs would be detonated from the safety of Solovetsky. It turns out Bell is not part of the black ops team originally, they were one of the passengers Arash Kadivar shot in Turkey, Bell is a Soviet spy associated with Perseus, Kadivar shot them due to Perseus favoring them more. Adler found Bell and brainwashed them to assist in finding Perseus, all the memories of Vietnam were implanted using Adler's own memories.
Bell/The player is given a choice, divulge the location of Perseus in Solovetsky or lie and claim he's at the Duga radar array.
If Bell lies the team heads to Duga and learns they've been tricked, if Bell arranged an ambush beforehand Perseus will show up with soldiers and the black ops team will be killed and Perseus will grant Bell the honor of detonating the nukes. If there's no ambush Adler will shot Bell for their treachery.
The warheads are detonated, millions are killed and the blame falls directly on the U.S. President Reagan disavows Adler's team and Operation: Greenlight while Perseus continues to spread Soviet influence and undermine the U.S.
If Bell tells the truth an assault is launched on Solovetsky, the team clears the air for an American bombing run to destroy the transmitters controlling the detonation. Perseus is still at large but it's implied he was acting rogue and the Kremlin will have to denounce him to avert suspicion and the C.I.A will continue their search for him. Adler takes Bell for a private talk, telling them they are hero and heroes makes sacrifices, Bell must make one more, Adler tells them it isn't personal, Bell is a loose end. They both draw guns on each other, the screen fades black as a single shot rings out.
