In the hallways of the St. Mary's Boarding School, new girl Irene goes to her class.
Madison sits at her desk and carves her name into the surface with a nail file. Irene comes in and the teacher, Mrs. Watson, introduces her, saying Irene will be joining them for the rest of the semester. The teacher has Irene introduce herself, and Irene awkwardly does so. She says that she's going to do a project on psychic abilities for the upcoming science fair, and the teacher tells her to take a seat. The others giggle at Irene, and one of them kicks Irene's desk as she sits down.
One of the girls tosses a note on Irene's desk, calling her a slut or a loser. Madison looks for a pencil, finds one where there wasn't one a moment ago, and hits Irene in the back of her head when she tosses it at her. The pencil lands on the floor and disintegrates into dust.
Later in her room, Irene sits among her partially unpacked bags and reads one of her books on psychic abilities. She hears girls laughing outside, looks out the window, and sees Madison with three of her friends. Irene goes to Watson teacher and says that she wants to change her fair project, but Watson tells her that the fair guides have already been printed.
Later in class, Irene displays the ESP zener cards and has everyone write down a symbol on one of the cards that she draws. Irene scores the results later and discovers that Madison has a near-perfect score. She goes to Madison's room, knocks on the door, and starts to walk away. Irene then comes back and knocks again, and when Madison answers the door the other girls tells Madison to bring Irene in. Madison asks what was wrong with her test, and Irene tells her that she got 0 on every test but it's not possible. Irene figures that Madison deliberately failed the test. She suggests that Madison is suppressing her psychic powers and if she had them, she could do anything. Madison is intrigued, and Irene says that she wants to do another test. The girl refuses and closes her dorm room door, and Irene leaves the zener cards on the hallway floor and walks away.
Later, when the girls in Madison's clique leave, Madison sees the pack of cards on the floor, picks them up, and goes back into her room. She experiments with them and identifies each symbol without initially looking at the card, and wonders what's going on.
Madison gives Irene a note telling her to meet her at an unused girls' bathroom. Irene does so and finds Madison there. Madison accuses Irene of pranking her and demands to know how Irene did it. Irene insists that there was no prank, and says that Madison is special because she has psychic powers. When Irene insists that it's cool, Madison tells her to stop talking and then asks what she does. Irene suggests that Madison figure out what powers she has and then they could figure out how she can use them. Madison agrees but warns that if it's a prank, she'll ruin Irene's life. After Madison leaves, Irene goes to the kitchen and grabs some supplies. She takes them to the bathroom and lights candles and calls Madison there.
Irene explains that she read that candles help someone focus their psychic powers, and lists the different psychic abilities. Madison tells her to slow down and Irene goes over the list. When Madison asks if Irene wants to be normal, Irene says that she does and Watson made her do the project on psychic ability. She uses the zener cards and Madison gets them right, and wonders if she's always been doing it. Irene figures that Madison can't feel her sixth sense but just has it, and every time she does it she gets better at it.
After setting the card on fire, Irene tells Madison that now she'll have to read Irene's mind to get the image. Madison does so and "sees" a star, and Irene confirms that she's right. They do more, and then Madison walks through the cafeteria with her clique and reads people's minds. Madison tells one girl, Debbie, to leave because they're sitting there. When Debbie hesitates, Madison threatens to tell everyone how she saw her father masturbating to QVC and watched him finish. Debbie quickly leaves, and bumps into another girl and drops her book.
Irene sees the whole incident, goes back to her room, and does her makeup. When she meets with Madison, the other girl notices and gives Irene some tips. They then go back to psychic testing, and Irene has Madison do telekinesis. Madison isn't able to do it, and says that she's done for the night and has somewhere to go. She invites Irene to come with her, and as they go upstairs Madison talks about how they used to hang out on the balcony and get high. One of the other girls, Gwen, had an older sister who told them about the bathroom. A teacher finds them and threatens them with demerits. Madison reads her mind and threatens to tell the dean she's been raiding the petty cash drawer for her gambling at the casino. Irene demands the key to the balcony as collateral, and the teacher orders them back to their rooms without giving them the key.
As Madison and Irene go, Madison says that it was baller and Irene tells her that it was worth a shot. A key appears on the hallway floor behind them and they hear it drop on the floor. Irene figures that Madison can conjure objects into existence, and Madison grabs the key and goes to get her clique friends, Gwen and Lisa. Irene hesitates and Madison tells her to come along.
The girls go out on the balcony and the key, once used, disintegrates into dust in Madison's hands. Irene notices and suggests that items Madison conjures only exist until they've served their purpose.
The girls drink, smoke marijuana, and talk about orgasms, and the other girls laugh at Irene. Madison tries to stop them, and Irene goes along with the other girls when they dare her to take her shirt off. When the other girls object, Madison reads their minds and insults them and Irene offers to go. She tries to get off the balcony and falls onto the concrete several stories below. When Madison goes down to check on Irene, Irene comes back to life and gets up, unharmed. She doesn't remember what happened, and wonders if Madison saved her. Irene figures that Madison has an unconscious psychic ability that she used, but Madison doesn't believe it and tells Irene to go back to her room before someone sees them, then runs off herself.
The next day, Madison's clique invites Irene over to their table to sit with them. They say that she should be dead, and admire Irene's new attitude about makeup and how she doesn't care that she almost died. Irene laughs along with them, while Madison watches from nearby.
Later, Irene parties with the other girls in one of their rooms. Madison waits for Irene in the unused bathroom, but Irene never shows up. She goes to the room and finds Irene, almost passed out from drinking. Madison takes Irene to the latter's room and tells her to stop hanging out with the other girls. Irene apologizes for missing training, and Madison says that she doesn't care about training or Irene. When Irene wonders if she's jealous of her hanging out with her friends, Madison says that they're not her friends and don't talk to her anymore. She wonders what she's doing at St. Mary's anymore, and tells Irene that the two of them are done, then leaves.
The next day, the girls tell Irene to visit their booth at the fair, and they have a surprise for Irene.
Madison lies in her bed and stares up at the ceiling.
At the fair, the judge dismisses Irene's science project as a medium effort. Irene goes over to the girls' exhibit and sees them doing an "Anatomy of a Loser" with a video playing that they took of her when she was drunk. Madison arrives and sees the video, and Irene screams in anger. The TVs shatter and the girls fall over, dead.
Later, the paramedics take away the corpses. Madison watches along with the other students, then goes to the bathroom where Irene is waiting. When Madison asks what she did to the girls, Irene asks her if she knew the girls were going to do that to her. She says she has no idea what Madison is talking about, and Madison says that it's been Irene's power the entire time and she's been manipulating Madison so she could become popular. She figures that the powers are Irene's, not hers, and Irene made her think she was special. Irene says that she is special, but Madison doesn't believe her and figures she has no powers.
Irene insists that the powers are Madison and she defended her, but Madison says that Irene ruined her life and she has no reason to defend her. Madison says that she wanted Irene to be a normal girl so she could have a friend, and Irene says that Madison is in pain because she won't admit that they're friends and that she saved Irene from people that were hurting her. Irene asks why Madison won't accept that they're friends.
Madison says that Irene will hurt or go away. Irene promises that she'll never go away, and Madison says that she's Irene's friend. She hugs Irene, and Irene disintegrates into dust... her purpose served. Madison crouches on the floor, screaming in horror.
Later, Madison goes to class alone. A new girl comes in and sees Madison, who finishes scratching "Madison + Irene: BFF" on her desk.
