- Ruka wore a revealing and sexy outfit, accompanied with leather shorts, yukata with her belly exposed and fishnet stockings.
- Graphic nudity from both genders. A working dildo is shown and implied that a charecter lost her virginity to it.
- Ruka is groped in the subway. We see the timid man carefully grasp her buttock which is defined by her leather shorts.
- A human-chair hybrid is seen, we see its bare nipples. She then urinates on a crowd (lasting for about a minute!). Men are then seen buying her. (implying sex)
- A woman who has an alligator mouth as a pair of legs, opens her "mouth" and we see her vagina.
- A man is shown with an enormous erect penis that he uses as a cannon.
- A quadruple amputee in fetish-like gear later gives a blowjob to her "owner". It is implied she has been his 'pet' for years. We do not see anything graphic, but the man spits and exaggeratingly shivers from the intensity.
- There are brief scenes of prostitution and several women in a club are wearing latex fetish gear.

- Very frequent and gory violence. Quite possibly, one of the goriest films ever made.
- The movie is very pervasive and graphic with violence from arms being amputated. The violence is portrayed similar to traditional Japanese anime and manga, with extremely exaggerated bloodsprays, eviscerations and body damage. An overlapping theme is that the culture of Tokyo has an open mind regarding self-mutilation - to the point that many advertisements are geared toward the 'cutter-culture'. One of the more graphic scenes is a mock-commercial in the movie that portrays a mentally disturbed man who guns down several schoolyard children - only to be shot and viciously mutilated by Tokyo Police Force via Katanas.

- Ruka goes to a bar, but only drinks orange juice.
- We see several Tokyo Police Officers holding bottles containing alcohol and it is implied they have beend drinking. The Police Officer mentions he wish he had some speed, then doodling a needle going into his arm with pen while jokingly imitating the effects of heroin. (perhaps falsey confusing it for speed.) The Police Chief later smokes a large cigar.
- A man injects himself with a bright green liquid three times.
Frightening & Intense Scenes

- Extremely explicit violent and sexual content.
- The film is exceptionally gory. Its all over the top and comedic though. Several gross scenes include humorous exaggerations of mutilation and mutations. A man eats several roaches and nasty looking worms casually on a train. A human-chair hybrid urinates viciously on an audience. The film is full of satire and parodies - alowing it to be taken light-hearted.

- A woman whose boobs have been cut in half and stapled back together, gives a man a blow job, and soon after bites his penis off. We see her spit out the penis on the ground.

- Very graphic scene of self harming (about 3 minutes in)
- After a Tokyo Police Officer is turned into an engineer and attacks his co-workers, the Police Chief orders for an extensive 'Engineer Hunt' - which results in the gruesome massacre of civilians and vulgar displays of power.
