- The Cooper family live in Pittsburgh and are happy together, with the father, survival expert Ray, and his teenage daughter Rachel sharing an interest in practicing mixed martial arts and self-defense at the gym Ray runs. However, things go downhill when Amanda (Adria Arjona), Ray's wife, falls ill with cancer. Ray leverages all the assets he has to pay for Amanda's medical treatment. Amanda is given a drug Infirmam from a company called BioPrime.
Ray Cooper (Jason Momoa) is told that a potentially life-saving drug (a generic version called Spero of a cancer drug from BioPrime) for Amanda, was in the final stages of FDA approval. This would bring down the cost of treatment tremendously and increase Amanda's chances of survival. Ray is devastated to learn that the generic drug was pulled off the market days before her treatment was set to begin, due to BioPrime CEO Simon Keeley (Justin Bartha) paying the manufacturer to delay production indefinitely.
Ray watches Keeley on a live debate with Congresswoman Diana Morgan (Amy Brenneman). Diana was debating against the licensing practices of big Pharma that make drugs very expensive and out of reach for patients. Keeley argues that drugs are created after extensive research and testing, which makes the process very risky and very expensive at the same time. The pills only cost $4, but Keeley says that he has to recover the billions of dollars spent on developing the drug and bringing it to market.
Ray calls in and threatens Keeley, saying if he doesn't reverse his decision, he'll kill him. Keeley doesn't take the threat seriously and Amanda passes away soon after, devastating Ray and their daughter, Rachel (Isabela Merced). Ray was training Rachel as a survival expert and was also teaching her boxing and self-defense.
Six months later, Ray receives a call from a journalist, Martin Bennett (Nelson Franklin), who tells him that he has evidence of criminal activity committed by BioPrime. Martin says that he saw Ray call Keeley on TV and offers to help him get justice for his wife. They meet on a subway, unknowingly followed by Rachel and a hit-man named Santos (Manuel Garcia-Rulfo). Bennett explains that BioPrime has been bribing anyone who questions their dirty deeds. Bennett says that Keeley has made payments to offshore bank accounts and to players at the state and federal level. Bennett says that he has enough material to prove that Keeley made payments to pull Spero from the market. But before he can share his information, Santos stabs him dead. As the train stops at a station, Santos stabs Ray and knocks out Rachel, leaving them both on the platform for dead.
Two years later, Ray has been obsessively tracking Keeley's movements. Ray poses as a waiter to infiltrate BioPrime's charity auction to announce the strategic partnership between BioPrime and US Government for cancer cure drugs, to cap the cost of treatment.
Ray infiltrates the event so he can interrogate Keeley. Keeley says he knows nothing and tells him to go after BioPrime's chairman, Vinod Shah (Raza Jaffrey). Keeley says that Vinod hired him, and nothing happens without his explicit sign off. After a brutal fight that leaves one of Keeley's bodyguards dead, Ray strangles Keeley to death with a plastic bag.
Ray then gets Rachel, and they hide in a motel outside of town after they leave the US and enter Canada. Rachel, concerned that her father has gone too far, contacts FBI Agent Sarah Meeker (Lex Scott Davis) and tries to convince her to look into BioPrime. The next morning, two mercenaries break into the motel but Ray sets a trap and kills them both, resulting in further tension between him and Rachel. Ray plans to go after Shah and lets Rachel help him trap Shah. Rachel uses a disposable cell phone to keep in touch with Meeker and provide her regular updates on what's happening with them.
Ray plans an elaborate plan to separate Vinod from his entourage, while he is driving back home through an abandoned stretch of the woods. Ray succeeds, but Vinod's car is then attacked by Santos. Ray tries to interrogate him to find out who the bribes were paid to), but Shah refuses to talk and is soon killed by Santos. Ray and Rachel manage to escape, but later meet with Santos in a diner. He had experienced persecution from an oppressive government in his homeland as a child, so admits that he is sympathetic to Ray's cause. Santos agrees to a temporary truce and reveals that Congresswoman Diana Morgan is his employer, giving Ray a lead on her location, but warns that the next time they meet, he will kill them.
Returning to the city, Ray is ambushed by the FBI, who got reports of a stolen tow truck near where Vinod was killed and tracks it back entering the city and flees to the roof of a baseball stadium. As Meeker tries to talk him down, it's revealed that "Ray" is actually Rachel. Ray died from his wounds at the subway and Rachel, suffering from PTSD and a Dissociative Identity Disorder (a mental illness that involves having multiple distinct identities or "personality states". It was previously known as multiple personality disorder), has devoted herself to avenging her parents' death, as she is broken after losing them both, several months apart.
She jumps into the river but gets knocked unconscious and put in an ambulance. After breaking free and crashing the vehicle, she locates Morgan's campaign office, where Santos is waiting. Despite him initially managing to subdue her by strangling and drowning her, Rachel is able to regain her strength and stabs Santos to death.
She confronts Morgan and secretly records her admitting she was bribed by BioPrime for government contracts as she needed the money for her campaign and that she ordered the hit on Bennett and Ray. After sending the recording to the FBI, Rachel obtains a fake passport, exchanges her money for Cryptocurrency, and boards a plane to an uncertain future. In the aftermath, Morgan is arrested for her crimes.
