Flipped shot: When the Enterprise returns to Gravesworld, the registry number NCC-1701-D is mirrored.
Data strikes Picard with his right hand from the lower left side of his body; from Picard's perspective, the force of impact is traveling from Picard's lower right to his upper left, but Picard spins a full turn to his right, stumbles to his left, and slams into a wall. Picard should have spun in the same direction as the force of impact.
The decision is made to do a near-warp transport, where the away team is beamed to the planet just as the ship goes back into warp speed. This is entirely unnecessary. The Enterprise had to go all the way to Graves's planet to be in transport range. The transport only takes a few seconds. So the Enterprise only saved maybe 1-2 seconds, while the transport could've gone horribly wrong and materialized the away team inside a wall.
Dr. Pulaski goes to the bridge via turbolift without once uttering her destination to the computer.
Lt. Selar plans to transfer Dr. Graves to the Enterprise "as soon as possible," even though his life expectancy is "a week." There is no explanation for the rush, let alone the transfer plan.
After Kareen asks Worf if he's a Romulan, Dr. Graves tells her that Worf is a Klingon and that Klingons and Romulans don't look much alike, even though they act much alike. However, he could have easily told her that Dr. Selar is a Vulcan and they're biologically similar to Romulans. It's interesting that she had no interest in Vulcans as if she knew about them already, but wasn't taught that Vulcans and Romulans are distant cousins.
