Sarah says to Ruth, "Help me I'm being held hostage!", meaning by Bettina (Kathy Bates). Bettina also says to Sarah "Do I have to carry you back to bed, or are you going to walk yourself". Additionally, Bettina is in complete control of distributing Sarah's medication to her, and a pig is referred to at one point. This is some of the many homages to Kathy Bates' famous Oscar-winning role, as Annie Wilkes in Misery (1990).
