Methemoglobinemia is a serious medical condition caused by elevated levels of methemoglobin, an abnormal type of hemoglobin (red blood cells). Hemoglobin is the iron based compound in red blood cells that carries oxygen, normal hemoglobin is made of ferrous iron (Fe2); methemoglobin is made of ferric iron (Fe3), another isotope of iron. The important distinction is that due to the different properties of ferric iron methemoglobin is not capable of binding to oxygen and carrying it through the bloodstream. Normally around one to two percent of hemoglobin in the bloodstream is methemoglobin, higher percentages are caused by either a rare genetic condition, overdose of certain medications or exposure to certain toxins. The most common agents that cause methemoglobinemia are benzocaine overdose (topical anesthetic), dapsone overdose (antibiotic used to treat leprosy) and exposure to nitrates (nitrogen based compounds typically used as fertilizer).
