- Kayako is covered in blood in the majority of the film.
- The heroine's fiance hits a cat with his car. He gets out to look at it (there's no blood or gore).
- A ghost child causes the heroine and her fiance to wreck their car, causing her to have a miscarriage (implied when blood trickles down her leg) and putting him into a coma.
- There is a birth scene with no nudity--the mother's knees are draped with a sheet. There is blood from the birth shown.

- A young woman finds her boyfriend hanging by the neck from the ceiling--he has been strangled by Kayako's hair. Shortly thereafter, the reporter suffers the same fate as her boyfriend in a somewhat graphic representation.

- A woman gives birth to the ghost of Kayako before passing out. Kayako emerges from between her legs and proceeds to back a man into a corner before the camera cuts to black. It is implied that he is killed.
