This is subject to interpretations, because no character ever really refers directly to this event. As some assumed than Ash´s "real world", in which she interacts with her pet, is as virtual as the game Avalon (that would be deducted, among other details, from the fact that the same sepia filter is pictured in both universes), the dog could be entirely a product of Ash´s imagination, one thing she unconsciously created as a motivation to live. His disappearance could also symbolise the beginning of her personal quest to level special A, as she "meets" him again inside (as a kind of picture presenting the opera named "Avalon") during one of the final scenes.
Of course, it is still possible the dog simply escaped, though this explanation seems too easy to be true regarding the complexity of the entire movie.
