- Sindel's outfit is rather suggestive in approach, particularly the low cut top that exposes quite a bit of her bust.
- Sheeva's outfit is little more than a pair of red strips of cloth covering just the center of her breasts down to the front of her lower body. She has a thong-like strip barely covering the center of her backside.
- Kitana's, Mileena's and Jade's outfits are much more cropped than before, leading to a lot of skin being shown.
Violence & Gore

- Like the first two Mortal Kombat games, this is a very graphic and bloody title.
- Kombatants still perform fatalities, often to graphic results, though some of them have a comical tone and may not contain as much gore as in Mortal Kombat II.
- The new finisher, the "Animality", showcases the characters turning into colorful animal forms and killing their opponent in a way specific of the animal (Liu Kang's dragon form bites an opponent in half, Sub Zero's polar bear leaps on an opponent and mauls them to death, etc.)
- Bloodshed still plays a prominent role in the game, this time in a few shades; many character bleed red blood, but Sheeva and Reptile bleed green blood while Sektor and Cyrax bleed black oil.

Frightening & Intense Scenes

- Some may still be shocked by the bloody nature of the game, even after 3 successive titles.
- Shao Khan's invasion leads to the stealing of billions of human souls, reducing the Earthrealm to a derelict, lifeless world with a black sky and perpetually burning red portal over his palace. Many of these souls can be seen in the Soul Chamber stage, roiling in agony in a soulnado.
