- A prostitute in her underwear is seen on top of a man just before a group of men bust in the room and stop any sexual activity from actually taking place. She's only seen from the waist up.
- Samantha/Charly is seen in the shower, her nude butt briefly show from onside.
- Samantha/Charly flashes Mitch. Mitch ogles at her nude body. Since they only show this from her back, no nudity is seen.

- Charly sneaks into an area, and she stabs and kills one guard with a knife. She shoots at several others, and falls into a cellar.
- A frozen corpse is seen a few times.
- There's an intense car chase in which several men pursue Mitch in a car and fire guns at him. Samantha shoots at his attackers, killing the driver, and causing the car to crash. Blood spatters are seen on the windshield.
- There's a large shootout in a train station, and several people are shot (bloody). Mitch and Samantha escape through a hallway, as a grenade explodes. They smash through a window and fall into a frozen lake as the building explodes.
- Mitch attacks Nathan Waldman, who knees him in the groin and knocks him over. Samantha helps attack Nathan, and he was knocked out.
- Samantha has a hallucination where her alter ego slits her throat (brief and not seen directly).
- Mitch throws a knife into guard's neck, killing him.
- Timothy stabs a man, though all that's actually shown his advance with the knife.
- Samantha Caine breaks the neck of a dying buck.
- Charly blows up a building, causing Mitch to be blown out a window, and several others to be apparently killed.

- In a wheelhouse, Daedalus tortures Samantha by holding her underwater via a large wheel. Later, she grabs a gun from a corpse underwater and shoots at Daedalus several times, killing him
- Earl and Samanthahit a deer, causing a violent car crash. Earl's head is seen bloodied with a glass shard in his eye as flames consume the car, and Samantha is seen being thrown from the car, with a large wound on her head.
- There is a violent fight in a family home, with Samantha's young girl placed in mortal danger. Samantha is hit in the face, thrown, choked and punched. She eventually managed to retaliate, violently smashing a glass dish over her assailant's face, then breaking his neck with her hands.
- In the flashback, Samantha/Charly takes a needle and stabs One-Eyed Jack in his eye, but then she is shot in the back.
- There's a shootout near the end in which Mitch shoots several men and navigates a base with many attackers while Charly watching through a sniper rifle guides him. Both shoot several men, and Mitch is shot a few times
- Timothy and Samantha/Charly fight with punches and kicks thrown (including Timothy slashing her with a knife, blood seeps from the wound). The two fall into a storm drain and fight while hanging onto fencing, and he falls.
- Timothy is shot by Samantha/Charly with an automatic weapon until he falls out of a helicopter and lands on a truck below.
