- A woman wears nightwear with slight cleavage.
- An old man seduces a young girl, but nothing happens. They kiss briefly at the end of the movie.
- A man kisses another man for celebrating their victory.
- A man says that the mangler needs virgins' blood, and another man jokes about examining a girl to find out if she's a virgin.
- A man tells to another man that he's a sexual maniac.
- An older man asks a 15 year-old if she's a virgin.
- It is implied that Lin-Sue was once a prostitute and is now having sex with Gartley, and while taking a bath she invites him to "keep her company".
- The word "pervert" is said twice.
Violence & Gore

- The violence in this movie is quite frequent, strong, and bloody. However, it is more graphic in the Unrated version than in the R Rated version.
- A girl cuts her hand and sprays blood on the mangler.
- An old lady gets caught in the mangler, and it slowly crushes her body as it drags her into the machinery. This is a very gory scene that is quite drawn out, as her work mates try to save her. The remains of her body are glimpsed at briefly in a crime scene context. We later see her face that has been reformed to the best of the morgue workers' ability.
- The mangler releases a steam hose that burns some of the female workers. We see the aftermath on one of the girls' faces later on, as she is in the hospital, suffering terribly.
- A man gets his arm caught in the mangler, which it proceeds to pull him into its gears. A co-worker is forced to cut his caught arm off with an axe to free him (graphic and gory).
- A man on his death bed coughs up some blood.
- A girl is hit over the head from behind with a wooden blunt instrument. She is then restrained and put into sleep via chlorophyll.
- A woman is pushed into the mangler, which pulls her in and mangles her.
- An old man is knocked onto some folding pads, which proceed to fold his body in on itself, forcing his limbs to bend and snap until he dies.
- A man is cut in half by the mangler (graphic and gory).
- A man punches another man.
- Stanner begins to get depressed after realizing that Gartley doesn't care about the deaths of his workers. He is threatened by Gartley and later dies.
- The mangler comes alive and literally starts hobbling around, chasing the three protagonists through the factory.
- Officer Hunton mentions that his wife was killed in a car accident. Sherry's parents were also killed in an accident.
- A little boy suffocates inside an ice box and his corpse is seen while his grieving parents cry.
Profanity

- Some uses of fuck and shit. Several references to God. Jerk, bastard, damn, and son of a bitch are also heard. A few sexual references, words like "pervert", "virgin", "dog-fuck". Some mild name calling like "I wish she'd go back into whatever sewer she crawled out of!" and "idiot" are also heard.

Frightening & Intense Scenes

- This movie is quite intense, dark and bloody, and rather crude at times.
- All the violent scenes are very gory and intense (especially in the R-Rated version), some of them also emotionally.
- The mangler's appearance is frightening, as it is a massive black machine possessed by a demonic force that devours people. Young kids will most likely be scared by the machine and the story.
- In one scene, a machine (that is not the mangler) gets exorcised, and frees some kind of a blue tornado. This, along with the whole movie, might scare people who are afraid of supernatural events.
- The old man's appearance is very scary and disturbing, and his acts and words are cruel and despicable. He is overall a disturbed individual that might scare young kids.
- The chase scene at the end is quite unnerving and scary, as it involves two men and a girl that escape from the mangler, which started walking and spurting fire, and is after them.
- The finale is rather shocking and unexpected.
- Sherry experiences grief over the deaths of Frawley and Stanner, whom she regarded as close companions.
- There are implications of Lin-Sue being a former prostitute.
- A little boy is killed, which might bother viewers who are sensitive to the deaths of children in films.
