The butler brings a tray into Dr. Willis's bedroom and places it on the bedside table. Before he enters, Dr. Willis is reading in bed, and the tray is already there on the bedside table. Moreover, the tray initially has a full milk glass and a white dish with two blue stripes, but when seen near the bed, the glass is only 2/3 full, and when Columbo arrives later (at around 26 mins), the dish is lower and clearly has only one blue stripe.
Dr. Willis' body is discovered at 1:10 a.m. When Columbo shows up at the house, he asks a policeman what time it is. He responds it is 1:30 a.m. It is highly unlikely that only 20 minutes elapsed from the time the body is discovered, the police called, two patrol cars and the coroner arrive, then Columbo, especially in a city as large as L.A.
When Grace is sitting up in bed and Ned enters her room and sits on the bed, she is propped up on a small pillow which is lower than her head height. When Ned is responding to her from a side shot, the pillow is massive and clearly well above head height,
The silver tray that the butler brings up to Dr. Willis, has a glass of milk and a blue and white dish with a pill in it. When Grace empties her pill into his milk, the dish is missing from the tray in several consecutive shots. Then, when her husband takes his sleeping pill a few minutes later, the blue and white dish reappears on the tray.
Columbo climbs the stairs drinking coffee from a cup in his left hand while holding its saucer and his cigar in the right, but upon entering Dr Willis' bedroom, the saucer has disappeared, and the cup is now in his right hand with the cigar in his left.
The episode of The Tonight Show Starring Johnny Carson (1962) the servants are watching the night of the killing aired June 27, 1975, which was a Friday (as Carson himself indicates on-camera when he wishes his viewers a nice weekend and discusses his upcoming guests the following week). However, the following day when Columbo goes to see Grace Wheeler, her Butler says it is Friday while announcing the arrival of her voice coach.
The plot pivots on the running time of Walking My Baby Back Home (1953), which Columbo asserts to be one hour and 45 minutes. The film actually runs for 95 minutes.
After forcing the gun from the dead doctor's hand, the doctor's fingers twitch on their own.
Columbo's dog is leaning out the car window and Columbo is waves the dog back into the car. The dog is heard barking but visually the dog is not barking.
When Grace arrives home from the premiere (at around 3 mins), she sweeps in through the heavy front door and pushes the door with her hand to shut it. There is a distinct closing noise, but the door never does actually close.
When the butler flies the American flag in the garden, he raises it halfway up the flag pole, explaining to Columbo that this is because of a death in the family. In fact, a flag being flown at half-staff is ceremoniously raised to the top of the pole for a moment then lowered to the halfway position as a mark of respect. However, in an unofficial setting such as a private estate, such formalities are usually dispensed with as they are considered superfluous.
Raymond the butler speaks in a very cultivated accent in some scenes (including informal scenes with his wife), but has a coarse cockney-type accent in others, later in the proceedings.
Sam Jaffe as "Dr Willis" is explaining to his wife "Grace" played by Janet Lee that he refuses to invest $500k into her theatrical production, he call s her " Rosie" .
