Abe needs to stay alive as he is in possession of a thumb drive with sensitive information about police corruption. - Abe Guevara (Frank Grillo) is caught in a shootout at a residential home that results in the death of District Attorney Joshua Gregory. He is chased on foot and attempts to escape with his brother Mateo (Christian Cooke), who he calls on mobile and they agree to meet at the rendezvous point in 2 minutes. Before Abe can reach Mateo, he is hit by a car. Mateo could see Abe lying on the road but had to drive away as the police arrive. Abe is subsequently sent to a local hospital.
A pair of homicide detectives, Regina Lewis (Marcia Gay Harden) and Masterson (Boris McGiver), are assigned to the case. They meet at the DA's home, investigating the crime scene. Lewis was at the crime scene before Masterson, and she claims that she tried to stop Gregory from bleeding to death, but she failed. Masterson tells Lewis that Abe and Mateo are mercenaries for hire and are currently freelancing for Big D. Abe owed Big D for a big drug score. Masterson reveals to Lewis that the guy shot, Gregory, was a DEA agent before he became the DA. Mateo was staring at 10 years in prison for drug dealing when Abe offered DEA a deal (in exchange for Mateo's freedom). A flash drive that would expose more than half of city's cops as corrupt and linked to the drug trade themselves. But the deal went wrong, and Masterson thinks Abe shot the DEA agent.
Paul Booker (Anthony Mackie), an every-man ER nurse, is assigned to oversee Abe's recovery at the hospital. As Paul checks in on Abe, he is attacked by a person hiding in the shadows. The mysterious attacker beats Paul takes his access card and escapes through the hospital window. Mateo (Abe's younger brother and partner in crime) is revealed to be the mystery attacker. Mateo is determined to break Abe out in order to pay off a debt with Big D (Markice Moore), an influential gangster. Mateo kidnaps Paul's pregnant wife Taryn (Teyonah Parris) and coerces Paul into working for him. Paul breaks out Abe on his stretcher and evades Lewis and Masterson just shortly after they arrive at the hospital to take custody of Abe and to transfer him to a prison hospital.
Abe drives Paul's car out of hospital, with Paul in the passenger's seat. Abe contacts Mateo and they again decide on a place to meet. Abe wants to ditch Paul, but Paul insists that Abe will not be able to survive without the medicines that Paul has been pumping into him. Abe arranges to meet Mateo at a bus station, but realizing that it is a sting operation, they incite a shootout and flee. Mateo drives away with Taryn, evading cops in a car chase. After fighting off more cops at a nearby car wash, Abe and Paul escape after hijacking a car. Evading the cops, the duo ditches the car and escape on foot.
Abe reveals to Paul that he is in possession of a flash drive, that can expose several corrupt police officers; while making the deal to distribute the flash drive to Gregory (the current DA and former DEA agent) they were ambushed, and Abe was wrongfully incriminated for his death.
Seeking a new car, they consult a local gangster, Cheetah. Masterson was tracking them. Lewis and Masterson arrive and hold them all at gunpoint. Masterson realizes that Lewis is one of the corrupt cops on the drive (when Abe says that Lewis was the one who killed Gregory) leading her to kill both Masterson and Cheetah. Paul kills her accomplice Jones while she is distracted with incoming reinforcements. Abe rendezvouses with Mateo, but he is ambushed for the drive as well by one of Lewis' men, who abduct Taryn. Abe and Paul arrive at the rendezvous, only to find Mateo mortally wounded. He succumbs to his injuries and dies. Lewis threatens the two, revealing that she is holding Taryn hostage.
Having taken into his possession security footage (from Cheeta's pawn shop) that incriminates Lewis of her corrupt actions, Abe finalizes a deal with Big D and pays off his debt (Abe had borrowed $60k from D to save Mateo and offers to trade his flash drive with all his evidence in return for D forgiving the debt and doing Abe a favor) Big D stages a distraction outside the police precinct, both to accommodate to Paul and Abe, but to also capture footage for a movie he is developing. Disguised as a first Responder and a police officer, Paul and Abe incapacitate Lewis, and save Taryn, who goes into labor and gives birth to their baby boy. In the aftermath, Lewis is killed by police reinforcements after Abe exposes her corruption.
One year later, Paul and Taryn celebrate their baby boy's first birthday, who they name Matty in Mateo's honor. Abe smiles upon seeing a picture of Matty's first birthday cake, and drives into the sunset, tailed by a black SUV.
