When Melinda has the vision of Gil coming to her house and sees the tattoo on her own forearm, it's her left arm she sees the tattoo on. Later, when she retells the story to Delia and talks about the tattoo, she points to her right forearm.

When Jim looks at Laura Bradley's autopsy report, he is interrupted by another doctor who calls him Jim. To everyone in Grandview, Jim Clancy died years ago and he is Sam Lucas.

By having Sam's middle name be James, the producers used this as a workaround for everyone to call him Jim. Even Jim/Sam's hospital white coat is embroidered "S. James Lucas M.D." - so his colleagues would probably know him as "Jim".

At the beginning, Melinda and Jim return from their date and find police cars and an ambulance in their driveway. Jim passes by a paramedic talking on his cell phone, who says "Hey Jim..." To the rest of the world, Jim is "Sam Lucas."
