During the conversation (then fight) between Malcolm & Dave Spath in the school yard, the children behind Malcolm repeatedly change position. Especially note the girl in the orange jacket with long blonde hair and the changing selection of kids immediately to her left.
Dave Spath has chocolate sauce on his face. Change camera to a view from behind him, he grabs Malcolm and some of the sauce is gone. Change camera to see Spath from the front and the sauce covers his left cheek again.
Malcolm's family is genuinely surprised when they receive the news that he is a child genius and has to be placed in advanced courses. However, later episodes show that Malcolm displayed giftedness since he was young, doing advanced robotics and playing with chemistry sets.
During the 'fight' in the school yard Malcolm covers Spath with chocolate sauce. After the fight, Spath backs away from the crowd. There are NO chocolate stains on his sweatshirt or face.

Malcolm slides to his left and the crowd of children moves with him. He slides right and ditto. However, when he stands to walk to Stevie there is not the expected proportionate movement in the crowd.
When looking at the picture Malcolm says the steering wheel is on the wrong side.

(at around 17 mins) When Malcolm opens his eyes in the Krelboyne class, lighting equipment can be seen on both sides. This is confirmed in the DVD extras by Bryan Cranston, which explains that they didn't care about the ratio change (from 4:3 to 16:9) during the pilot shoot, which explains many goofs.
In the pilot, when Lois is doing the laundry and the doorbell goes, as she goes to answer the door you can see someone move the laundry basket out of her way and try and hide behind a wall.
In the last scene of the episode, after zooming out to show the trashcan, there appears to be a crew member visible on the right of the screen.
Hal comes in after buying chocolate from Dewey's friend. Jamie is in his high chair. Then the camera cuts to Lou's and instead of Jamie a doll is clearly visible.
In the scene where Stevie is telling a joke, the back of the director's head and briefly fingers can be seen in the lower left in the shots facing Stevie, clearly directing him. This is confirmed in the DVD extras by Bryan Cranston.
