Under the remote supervision of James Bishop and his team, the mission is to locate and destroy shipments of Caesium before the highly radioactive substance can be weapons. In the ensuing firefight, most of the Russians and Overwatch operative Greg Vickers are killed, but critical intelligence on the Caesium is destroyed. Silva executes the last survivor, a teenager who warns him that he is making a mistake. Sixteen months later, in the Southeast Asian country of Indocarr, police officer Li Noor surrenders himself at the US Embassy to negotiate for passage out of the country in exchange for information. Noor is revealed to the asset of Overwatch agent Alice Kerr and reveals he possesses a disc containing information on the Caesium, and is able to negotiate passage out of the country in exchange for unlocking the disc, which is programmed to self destruct within four hours. Ambassador Dorothy Brady is visited by the country's Deputy Foreign Minister and state intelligence agent Axel, with the two demanding that Noor be handed over as he is wanted for espionage. Meanwhile, Noor fends off an assassination attempt by disguised government agents in the embassy infirmary using his skills as a former member of the elite tactical squad..

- American black operations agent James Silva (Mark Wahlberg) leads a strike team code-named Overwatch to infiltrate a Russian FSB safe house in the United States. Under the supervision of James Bishop (John Malkovich), Overwatch's mission is to locate and destroy shipments of Caesium before the highly toxic substance can be converted into a weapon to kill thousands. The team infiltrates the house and captures the FSB operatives. The team gathers all the intel including the DNA samples of the FSB agents and a tranche of SSB drives. The FSB agents counterattack and set the house of fire. The US team successfully kills the occupants, while Overwatch member Alice Kerr (Lauren Cohan) is wounded.
One of the Russians, an 18-year-old named Anatole Kuragin, falls out of a window during an explosion. Silva executes the boy who pleads not to be killed, and everyone escapes. From the materials seized from the safe house, the team starts tracking all the Caesium, but one last tranche seems untraceable. James Silva is a decorated veteran from several tours in Iraq. As a child, James was considered to be Hyperactive and genius. His mother and twin brothers died in a car crash when he was a kid, and James was the only one to survive.
Sixteen months later, the team is in Indonesia tracking down the last tranche of Caesium, But the information turns out to be wrong as the shipment they intercept only had some arms and fake paintings, and no Caesium. The source who gave them the information, was Alice's asset. 6 sets of Caesium, with 11 discs per set, are still missing. 66 missing discs means 66 dirty bombs. Alice says that her asset has been an excellent source so far, but Silva is angry as they came up empty.
Senior Lieutenant Aleksander Aslanov (Nikolai Nikolaeff), a Russian armed forces officer launches an operation from a plane with electronic spying and communication capabilities. With him is a woman to whom he assures that he would have failed if their target is alive in the next 24 hours.
Indonesian police officer Li Noor (Iko Uwais) surrenders himself at the U.S. embassy to negotiate for passage out of the country in exchange for information. Noor is revealed to be Alice's asset and reveals that the disc contains the information regarding the last 6 sets of Caesium, but he negotiates for passage out of the country in exchange for the code. The information in the disc is destroying itself and will be rendered useless in 8 hours. While Noor is being tested (he says that the confiscated paintings have Caesium disguised as the paint, which turns out to be true), Alice tries to come to terms with her family issues. Silva wants Alice to sort out the issues with her ex, so she can focus on the mission at hand. The ex-husband Luke wanted to know if Alice would be home at their daughter's school for Mother's Day. Luka is marrying a woman named Diane, who would be the stepmother to their child.
Axel (Sam Medina), leading a team from the Indonesian State Intelligence Agency, arrives at the embassy and demands that Noor be handed over. Meanwhile, Noor fends off an assassination attempt by Indonesian agents inside the US embassy compound. Overwatch operative Sam Snow (Ronda Rousey) and Alice arrive, shocked at his combat prowess, learning that Noor used to be a member of Indonesian Special Forces.
Silva agrees to take Noor to an airplane at an airstrip 22 miles away, as Noor will unlock the disc only when he is on the plane. While they are driving, Noor reveals his reasons for turning on the Indonesian government: The government had been corrupted and killed his family. While driving, the camera Bishop was looking at blacks out, then comes on again. During the camera blackout, Axel has his men put bombs on the car, making it explode. While Silva's team helps fend off Axel's men, Sam is mortally injured. After defeating most of Axel's men, Silva gives Sam two grenades, and leaves her, letting her suicide-attack the remaining henchmen.
Silva, Noor, Alice, and Douglas (Carlo Alban) enter a restaurant. Silva sees Axel, and confronts him, even though Bishop tells him not to. Axel tells Silva to give up Noor, but Silva refuses, and walks away. Silva brushes past two girls and realizes that there is a grenade in the restaurant, and tackles down civilians before it explodes. When the dust clears, Douglas is severely wounded, and Silva is attacked by the same girls, only for Noor to save Silva by killing them. They drive to a safe-house, where Douglas dies while holding off Axel's men.
While entering the safe-house, Alice is separated and meets a girl in an apartment. Alice and the girl escape harm by using booby-trapped grenades. Meanwhile, Silva and Noor split up, fighting Axel and his henchmen respectively. Silva and Noor meet up again and meet the girl Alice saved. She leads them to Alice, who is nearly killed by Axel's henchman before Noor intervenes-saving Alice and killing the henchmen by shooting him in the head.
On the way to the airstrip, the remaining team members are briefly confronted by Axel. Silva has Overwatch kill Axel by destroying his car with a "Hand of God" drone strike. The team barely make it to the airplane, where Noor and Alice board the airplane to meet her family once again. While on the airplane, Bishop notices Noor's heart rate is accelerating and it is revealed that Noor isn't a double agent, but a triple agent working for the Russian government, and the boy Silva killed (back in the FSB hit) was the son of a high-ranking female Russian general.
The official hired Noor to give Alice the wrong information so they would trust him, and just as Alice is realizing this, Overwatch is ambushed, and the entire team is shot. Bishop barely escapes, and rests outside while Alice's fate is left unknown. Silva realizes this too late and tells his experience during a post-mission debriefing.
Back at home, Silva puts up Noor's picture, vowing revenge.
