- The unrated DVD contains three additions that weren't in the theatrical version:
- 1. A close-up of the German Surgeon's severed leg after it's cut off by the chainsaw.
- 2. A slightly extended take of Kana's eye being cut off and the subsequent pus.
- 3. A close-up of the train crushing Kana's head during her suicide.
- In the German version, the dialogue that was in German has been changed to Spanish.
- The film was heavily cut by the Censor Board of India of 4 minutes to reduce all scenes of violence, torture, sex and nudity for an 'A' (ADULTS) certificate to the distributor 'Columbia/Tristar Films of India'.
- The film got a Limited certificate in South Korea, it was cut by 4 minutes for violence for an '18'.
- In addition to the R-rated theatrical release and the Unrated DVD mentioned elsewhere, there is an Extended version, which has a different ending, more or less horrible, depending on your point of view. The difference in runtimes among the three cuts is less than one minute.
- Beside the German, Indian, and South Korean cuts mentioned elsewhere, there is also a version one minute shorter, cut for release in the Philippines.
