The table that Bain shields himself with when the apartment explodes disappears when he falls out the window. Also, in later scenes his hands (especially his fingertips) appear uninjured; the explosion would have badly burned them at the least.
Miguel's hair is substantially longer in the final scene compared to earlier when he was camping at the hotel window (supposedly earlier in the day).
In the scene where Miguel is in the tower and setting up to shoot Rath coming out of the bank, with his left hand he pulls a bullet out of a holder that is strapped to his right wrist, yet when he loads the round in the weapon, he loads it into the chamber with his right hand. Him having the bullet holder strapped to his right wrist would be also be an incorrect procedure, as he would have to pull the bullet out with his left hand, then switch the bullet to the right hand to load and close the bolt. This would take up too much time.
The shape of the tree when viewed from the hotel remains the same even when the scene is viewed in flashback. Presumably there would have been some growth over the years.
When Rath and Elektra first drive up to the old hotel in the station wagon taxi, after they get out Elektra is heard saying "keep the air on". When they drove up, the windows in the car were open, and both were sweating. They wouldn't have opened the windows if the a/c was on.

In every scene where Rath and Bain communicate with the contractor through their computers, the sound of the keystrokes, as well as the positioning of their fingers, don't always match up with what's appearing on the screen.
(At 1:13:05-1:16:00) Rath originally gets on the Seattle monorail at Westlake Center terminus, makes the exchange at the Seattle Center terminus and gets back on the monorail. He jumps off on to the track buttress by exiting the right side of the train (in correlation with the train's motion). This means that the train is going in the wrong direction as the buttress is on the left side when traveling from Seattle Center to Westlake Center. Further proof is the exterior shot of the monorail heading toward the Space Needle after it resumes operation.
After the cemetery gun battle, the police arrest Miguel but follow no other procedures. Since a murder was committed, which makes the cemetery a crime scene, they should at least detain witnesses, including Rath, for questioning.
While Rath and Bain are fighting in Electra's apartment, Electra is driving away. She passes her neighbor Jennifer, who recognizes her and says hi. When Jennifer gets home and discovers the surveillance system, she somehow realizes it belongs to Electra and goes to confront her, even though she just saw her on the road and thus knows she isn't home.
Rath and Bain are considered the best and second-best assassins in the business. Yet throughout the entire film they make very little effort to be inconspicuous - for example, touching multiple things at job sites without gloves, which means fingerprints.

Outside of the apartments, Miguel puts his hands on the hoods of the cars to check for one that was still warm but the one he stops on is the first one he checks that was in the sun so of course it was going to be warm regardless.
Rath removes his jacket, causing the gun in his shoulder holster to be visible, before closing the curtains in the hotel room. Anyone passing by might have seen him wearing it.
