Larry, Glen, and Ray are in a cage in the basement of the Inferno. They hear the crowd cheering outside, and Larry sarcastically asks Glen if he thought New Vegas was everything he thought it would be. Glen says it's a bunch of lost, scared people, and Flagg is someone who makes them feel a little less lost. Ray says that the residents are evil, but Glen disagrees. Glen is happy to see the crucifixions on the street, because he figures Flagg can't count on his own people and makes public displays out of people that defy him
In the desert, Stu lays at the bottom of the crevasse, his leg broken. He's managed to start a fire, and lies with Kojak. Stu shivers with the cold and pain, and considers the bottle of pills that Glen gave him. After a moment he pours some into his palm, and Kojak whimpers and looks at him. Stu finally decides not to kill himself and puts the pills back in the bottle.
Lloyd and Rat Woman begin the trial of Larry, Glen, and Ray, in a courtroom. Lloyd says that they're accused of being spies, and they've spent other spies among them and the spies have been killed. He announces that Flagg has decided that they will have a fate at a different fate if they renounce Abigail and wears loyalty to Flagg. The crowd cheers, and Lloyd says that Abigail wouldn't defend them if she was there instead of them.
After Lloyd invites them to come over to their "good side", Glen asks how long the trial is going to go on and under whose authority it's under. Rat Woman addresses the surveillance camera and loudly says that the court is under Flagg's authority. Glen says that Flagg is the Mother of Dragons. Rat Woman throws a glass at him and tells him to shut up, and Lloyd tells Glen that Flagg is watching them.
In his penthouse, Flagg levitates and watches the trial on a big-screen TV.
Glen asks Lloyd why Flagg needs the trial and all of the adulation is he's so powerful. He asks why Flagg won't face them himself, waves to the camera, and invites Flagg to come down and talk to them. Lloyd punches Glen and tells him to show some respect. Glen says that all Flagg has over them is fear and they're all afraid of him, and tells Larry and Ray that Flagg is more pathetic than they though.
Rat Woman tells Lloyd--the bailiff--to shoot Glen. Lloyd tells Glen to stop talking, but Glen doesn't believe Glen will shoot him. Rat Woman has Lloyd approach the bench and tells him that he's embarrassing them. Lloyd turns and draws his gun, and Glen points out that his hand is shaking and he's never killed a man before. He says that Lloyd doesn't have to do it, and all he has to do is tell the people that Flagg is nothing without his fear. Lloyd shoots Glen dead and as Glen lies dying, he tells Lloyd that he forgives him because he doesn't know any better. Furious, Lloyd unloads the rest of his clip into Glen, and the broadcast goes off the air. He tells the audience to stop staring, and orders the prisoners out and the prosecution rests.
Flagg drops to the ground, his power waning.
The guards lock Larry and Ray up in a kitchen, and Ray tells Larry that she can't stand pain. They sit next to each other, and Ray wonders what it's all for. Nadine and her escort Wes come in, and she has Wes take Ray away. Wes takes Ray out, and Nadine tells Larry that Flagg called and she came, and now she's carrying Flagg's prince.
Nadine asks if any of them made it, and Larry figures that Harold set up the bomb. She doesn't want Larry to get hurt, and says that it took her coming to New Vegas to understand who she really was. Larry finds a pan to use as mirror to show Nadine herself, and she finally does and stares at her gaunt image in shock. The baby convulses in her womb and she falls to the floor, saying that the baby is coming.
Lloyd comes with a wheelchair and takes Nadine to the penthouse as she screams in labor pains. Once they get to the penthouse, Rat Woman prepares to cut open Nadine to deliver the baby, while Flagg looks on, smoking a cigar. Nadine yells that it doesn't feel right. The others ignore her, and she looks at Flagg and says that he knew he wasn't meant to survive breeding. Flagg assures her that she's his queen, and the black stone on a necklace around Nadine's neck glows.
Flagg looks at the stone and smiles, and Nadine seems to accept that she'll be Flagg's queen. As he walks away, Nadine says that Larry was right, rips the necklace from her neck, and drives the stone into the window. It shatters and Nadine falls to her death. Flagg screams in rage and frustration, then tells Lloyd to cancel the nursery.
In the desert, Stu sleeps and Kojak perks up as a wolf enters the crevasse. Kojak attacks it, and Stu wakes up when he hears the noise. Stu is unable to move because of his broken leg, and a blood-covered Kojak comes back to him.
Lloyd comes back to the top floor and finds Rat Woman in the hallway outside the penthouse. He asks if Flagg is still in the penthouse, and wonders what they're supposed to do now. Rat Woman tells Lloyd that it has nothing with her, and points out that he shot Glen. Lloyd says that he keeps seeing Glen's face in his mind and the look Glen gave him before he died. He breaks into tears, and Rat Woman tells him that he has to keep it together. Rat Woman tells Lloyd that they have to keep on task.
Flagg comes out in the hallway, and Lloyd asks him how things are going. He wishes that Flag would have told him if he didn't want him to kill Glen, and shows Flagg the necklace and reminds him of how he's been with him since the beginning. Lloyd claims that Rat Woman told him to kill Glen, and Flagg merely asks if the execution is ready to be broadcast. Rat Woman and Lloyd assure him that everything is ready, and Flagg tells Lloyd that there's one thing he wants him to do first, and "waste not want not".
Later, Rat Woman dresses as a bellhop and wheels a covered platter to Larry in the kitchen. She removes the cover, revealing Nadine's severed head, and says that it's compliments of Flagg.
Later, Lloyd and Wes come in and Lloyd tells Larry that it's time for him to live the rest of his life. Larry smiles and says that they brought Nadine's head to him to show him that there's no hope. He figures New Vegas is falling apart and Flagg lost his queen and his child. Larry says that Lloyd will be next, and Wes puts a hood on Larry's head and leads him out to where Irene is waiting, similarly attired.
Lloyd and Wes take Larry and Irene to the Inferno's pool, and remove their hoods. A crowd as gathered around the pool, and they chant "Make them pay." They're chained to the bottom of the empty pool, and Julia announces Lloyd. Lloyd steps out in front of the crowd, while Larry and Ray hold hands.
Julia "interviews" Lloyd about how the prisoners about feeling about being executed. Lloyd finally says that they're feeling scared, takes the microphone, and tells the crowd that Flagg will shake the world. He walks away.
In the penthouse hallway, Flagg hears the crowd cheering his name. He steps out on a balcony and he yells that they're heading to a future that is under threat because of a new infection. People who sent Larry and Ray don't want the residents to enjoy the freedom the world has to offer after Captain Trips., and want to chain the residents in their "invented morality".
Flagg continues, saying that the deluded fools worship "an old witch". He insists that there was never enough to go around even before Captain Trips, and the residents are the predators and the Boulder people are the prey.
Lloyd goes to a water valve and starts filling the pool, and Flagg dances as music plays. The crowd dances and cheers, and Flagg says that first they'll drown the rats and then they'll burn the witch. Rat Woman cuts the broadcast to the airport, and Flagg says that a plane is being readied to carry a nuclear bomb to Boulder and wipe it off the face of the earth.
Larry tells Ray that Flagg doesn't know that Abigail is dead. Ray wonders what it means.
Flagg tells the crowd that one of his favored servants is getting the bomb for the plane to deliver. Lloyd wades into the pool, extends a baton, and asks Larry if he has any last words. Larry says "I will fear no evil", Lloyd tells him to shut up and beats him, but Larry keeps saying the same words.
Trashcan Man drives into New Vegas with the warhead loaded in the back of his vehicle.
Flagg watches, smiling, as Lloyd continues beating Larry. A woman takes up Larry's words, and Flagg angrily asks who said it. Once they spot the woman, Flagg tells Lloyd to shoot her. Lloyd refuses, and Flagg yells that he's weak. The Head Custodian repeats "I will fear no evil" and the guards beat him down.
Trashcan Man drives into the Inferno, as storm clouds appear overhead.
Flagg tells Lloyd to do as he's told. He yells to the guards to get the keys and release the prisoners before they drown. Flagg laughs at the revelation of more traitors, while a lethally irradiated Trashcan Man drives the warhead to Flagg as the residents stare at him in horror. Flagg sees him and says that he shouldn't be there, but at the plane. Trashcan Man swears his life to Flagg, and the storm clouds gather overhead and lightning forms.
Clouds gather around the Inferno and pour into the atrium. The residents stare at the clouds, and lightning shoots down and shattered the windows and sets banners afire. The residents run and Flagg yells at them to return, but they ignore him. Julia runs and a lightning bolt strikes her, killing her. The doors don't open, and people are crushed in the stampede. The glass finally breaks and the surviving residents get out.
Rat Woman is blown apart by a lightning strike, but the lightning never hits Larry and Ray in the pool. Larry tells Ray not to be afraid and holds her hands,.
Lloyd is hit by a falling ornament and decapitated. A ball of lightning forms in the atrium in front of Flagg, and Flagg cowers before it. The water rises above Larry and Ray's heads. Meanwhile, Flagg stands up and faces the ball lightning, and bolts of lightning hit him repeatedly. More bolts hit the warhead and Trashcan Man, killing him as well. The warhead explodes, wiping out New Vegas.
Stu sees the glow of the blast over the edge of the crevasse, and the blast wave sweeps over the desert above. Kojak runs off and finds Tom walking nearby, and brings him to Stu.
In Boulder, Frannie is leaving the school with Joe, and they and the others see the glow from the explosion. Joe tells Frannie that Flagg is gone, and Frannie asks if he can see Stu. She clutches at her stomach as she has labor pains,
