- Heavy amounts of violence and especially gore persist throughout the film. Multiple characters are shown missing limbs, having body parts blown off of them violently, and being shot and stabbed.
- This is probably the goriest TF2 movie. Its ranged from mild blood to extremely graphical dismemberment. Most of the death scenes are heavily unsettling and shocking. Gore is present in almost the entire movie.
- A man is struck in the leg with an axe, which is then plunged into his neck off-screen (blood spray is seen) before the camera shows his decapitated body. Later his severed is used to trick a character before the axe is stabbed into his chest and he is shot in the head (a wall obscures the death but his blown apart head is seen).
- A man is tortured to death with a drill. The death isn't seen but the sounds are heard and are very disturbing.
- A woman's decapitated head is briefly seen along with a blurred-out shot of her body. Although not graphic, this is extremely disturbing and sudden.
- A man strangles another man and smashes his head against a trash can but before he can kill him, he is shot in the chest by another man with a pistol. Mild blood is seen.
- A man is shot repeatedly with a machine gun, causing his torso to be torn apart and blood to splatter everywhere. He begins to laugh and is killed by being shot with a shotgun although this is off-camera, with a brief shot being shown of blood seeping from his destroyed head.
- A man falls and hits his head on countertop, with this implying to have killed him.
- A man shoves a crowbar into a zombie's head and drives it in further. This is extremely graphic with blood splattering from the wound.
- A man's eye is gouged out with a screwdriver, blood is shown spraying everywhere. Non-fatal but bloody.
- A large man grasps the head of another in his hand and squeezes, crushing it. This is very brief and not graphic as it is seen from a distance although some blood is shown seeping through the fingers.
- A man is shot in the head and begins to convulse from the wound in his skull before turning into a zombie, very surreal.

- Frequent use of hell. Bastard, ass, damn, and bitch are also used.
Alcohol, Drugs & Smoking

- There is frequent smoking of cigarettes and alcohol consumption throughout the film. Parts of the plot center around the use of a pharmaceutical drug produced by a fictional company. Pickle.
Frightening & Intense Scenes

- There is an intense amount of unsettling imagery throughout the film, as well as sudden gore and violence which may catch some watcher off-guard.
- Scenes with zombies, mutants, deformed creatures, etc are present throughout the film. Some of the scenes which they're in are very intense.

Violence & Gore
- Cyclops (Demoman) is seen missing an arm with a cartoon-style open wound. He is also seen frozen and is blown up off-screen attempting to murder-suicide another character.
- A group of zombies are killed in violent ways. They are shot, electrocuted, burned, and one's skull is crushed by a falling cabinet with blood splatter shown.
- The detective is shown executing three people with bags over their heads by shooting them in the head. While the deaths aren't fully shown, this is still very disturbing.
- The corpse of a protagonist is seen, having been frozen to death. Not graphic but very upsetting and saddening.
- The detective is badly disfigured after an explosion and his entire face is burned off. He confronts Archibald and kills him by shooting him in the head, this is not graphic but sudden.
- The ending sequence is very dark and upsetting with both Fritz and Jane going on a rampage. Fritz kills several innocents by shooting them in the chest with blood spray shown and also kills two major antagonists while Jane also kills one.
- The detective forces Fritz to play Russian Roulette and he is killed when he accidentally shoots himself in the jaw. Blood is briefly seen spraying onto a wall but the camera cuts away, very dark but not graphic.
- A man is quite suddenly shot directly in the head with some blood being briefly shown spraying onto the wall next to him. His killer then stands up and shoots his body one more time.
- While in his car, a badly wounded man is shot to death. While at first no injury detail is seen, he is later shown with multiple bullet wounds seeping blood in the head and the face. As a man looks into the window of the car, he is hit by an ambulance; this is sudden but not graphic, with some blood being shown on the windshield.
Frightening & Intense Scenes
- Fritz is revived after his death and attacks his killer, stabbing him in the chest with a broken bottle with lots of blood spraying out before shoving his torso into a piano. Fritz smashes his head three times with the piano lid and although the death isn't shown, blood splatters every time the lid opens.
- In a hallucination, Ludwig (Medic) takes a prescription drug and the view suddenly shifts back to the real world. It is revealed Ludwig is dead with a sudden close-up of his face, eyes gouged out and teeth missing in a disturbing smile. This shot is extremely graphic and horrifying.
- There is a scene where a mutilated and deformed Jeremy (Scout) attacks Jane (Soldier) and Cyclops (Demoman) while loud noises play and lights flash. Jane violently stabs it through the head.
