Although Hugh Laurie's authentic portrayal of the American Dr. House was so convincing to audiences, those who didn't follow his work were surprised to learn that he is, in fact, English. However, actor David Anders (who portrays the patient in this episode) can be considered Laurie's "American equivalent," as he is often cast in the roles of Englishmen leaving many to believe the actor to be British.
House says, "If Park is right, he goes all cuckoo's nest on us." This refers to "One Flew Over the Cuckoo's Nest," a 1962 novel, later adapted to film in 1975, which features a sane criminal who feigns insanity to avoid hard labor at a regular prison.
