Charlotte's patient, Jane, is scheduled for gender assignment surgery, and Sheldon has to assess her mental stability and make sure she's a good candidate before she goes under the knife. Jane tells Sheldon that she wants to be a woman so that the outside will match the inside. Sheldon has his doubts about Jane's mental well-being because she attempted suicide, which suggests deeper problems and that the surgery won't help. Sheldon tells Charlotte that he doesn't think Jane is ready, and Charlotte suggests that maybe Sheldon's going against her because she's with Cooper and not him. Sheldon says that's the the reason.
Charlotte speaks with Jane and then argues on Jane's behalf to Sheldon, who's still reluctant to give his okay. Later, Jane is rushed to the hospital after she attempts to perform the surgery on herself. Sheldon tells Jane that he'll help her, and Charlotte and Sheldon start to smooth over their rocky friendship.
Pete's patient, a woman named Sydney, is prescribed medical marijuana. However she gives her pot to her son (Zack, Cooper's patient, who is autistic) in order to help him with his spastic fits. Cooper is frustrated with Pete because Pete's patient is giving weed to Zack, but it seems as if the prescribed pot isn't helping Zack anymore. In a desperate move, Sydney gives Zack some weed she bought off the street, but it's been laced with PCP.
Pete is annoyed with himself for prescribing the pot and putting Zack's life in danger, but Cooper points out that Pete couldn't have predicted what Sydney would do with her marijuana. Cooper says he's going to keep his time with Violet to a minimum in the future, and now the police have to be notified about what Sydney did to endanger her son's life. Cooper will stay with Zack and explain the situation, and Pete agrees to stay as well.
Addison finally tells Naomi about her her feelings for Sam. Naomi is in shock but that might just be fallout from the roller coaster ride following William's death. Naomi tells Addy that she and Sam need to support the merger, and that Addy must never bring up Sam in any conversation Naomi has with her.
