This was an attempt on the part of the writers to not let the son become the focal point of the whole series, although he is tangential.
As usual it has its plot twists; the scripts love to take the viewer down the wrong path until the last 10 or 15 minutes. In this case, a death was not a death. But, as in other episodes there were a few problems.
Jim died, was buried, and that should have officially been the end of it. It was clear med school was achieved on the transcripts of Sam. He would have to have enrolled as Sam, graduated as Sam, gotten his license as Sam, and been hired as Sam. He would still be appearing to everyone else as Sam. Yet his hospital coat has Dr. Jim Clancy sewn on it.
Further, the last line of the show uttered by Melinda is "So who is the morgue ghost?" with a jump back to that entity. That is never answered in subsequent episodes.
