(at around 33 mins) When the 12 chariots are parading to the final stopping spot, the fans are throwing flowers and all kinds of things on the road that they ride in on. When they show the overhead view and the last chariots pull up there is not one item on the roadway.
(at around 55 mins) When Katniss is turning around to show her fire dress, you can see her hair (bun) comes apart. But in the next scene her hair is nicely tucked in.
(at around 41 mins) When Peeta throws the metal ball at the spears, the career tributes (Cato, Marvel and Clove) are laughing at Peeta before he throws the ball. After Peeta has thrown it, Clove has gone and Glimmer is in her place.
After Katniss blows up the stockpile of goods, in all future shots of the same clearing there is no indication that an explosion took place or that the mines have been dug up.
(at around 15 mins) During the Reaping, when Katniss yells, "I VOLUNTEER!" her hair is messed up and ruffled, however, when she states "I volunteer as tribute," her hair is combed again.
(at around 1h 55 mins) When Katniss gets the cut on her head from the thrown knife, the wound has barely bled by the time she gets back to Peeta. Even small wounds in the head cause large amounts of bleeding very quickly.
(at around 1h 40 mins) According to the novels, the three-finger salute is given with the left hand. After Katniss constructs her memorial to Rue, she makes the gesture at the camera with her right hand, and the citizens of District 11 also respond in kind with their right hands. This is the only time a right-handed salute is used in the series.

(at around 20 mins) Effie says that the train travels at 200 miles per hour and that the trip from District 12 to the Capitol is "less than two days." If District 12 is described to be in the Appalachian mountains of today, and the Capitol in the Rocky Mountains, we can assume they have around a 1,500 mile journey, give or take a few hundred miles (around 2,400 km). 1500mi/(200mi/hr) = 7.5 hours, not even a whole day.
(at around 1h 22 mins) One of the tributes holds a metal sword in a campfire. It comes out glowing bright yellow. Campfires are nowhere near hot enough to make metal glow like that.
(at around 16 mins) After Katniss volunteers for Prim, it shows the crowd putting three fingers over their lips and kissing it as a sign of respect. You can clearly see a man in the crowd wearing black with a camera. This is often mistaken as a goof but remember that the 'Reaping' is being televised across Panem.
(at around 1h 13 mins) It is still daylight in the arena when Katniss climbs into the tree. It then pans across the control room and the Gamemakers broadcast the Fallen list shortly afterwards, at which point it's night. As shown later, the Gamemakers are able to control the environmental settings at the push of a button, so they likely switched the arena to "night mode."
(at around 1h 40 mins) After Katniss frees Rue from the net and kills the boy that throws the spear. The cannon does not sound nor does it sound after Rue dies.
In Katniss' flashbacks to Peeta feeding pigs at the bakery, Peeta's hair is Josh Hutcherson's brown rather than Peeta's blond.

(at around 15 mins) During the District 12 reaping, Effie Trinket is the only character with a microphone. However, after the female tribute has been chosen, Katniss and Primrose's voices can be heard amplified and echoed as if they were using microphones as well, even though neither are near the microphone. This is only in some shots, and the echoes come and go.
(at around 19 mins) You can see Gale standing outside the doorway waiting for his cue to come in to say his goodbye to Katniss. After her mother and sister leave, she tries the door and opens it a bit and then closes it. As soon as she closes it Gale opens it. You can see Gale standing there when she opens the door.
(at around 1h 14 mins) The first night when the images of the fallen tributes are shown in the sky, the District 9 boy and District 10 girl's faces are not shown in the sky, despite dying in the opening bloodbath. The District 9 girl's face is the last one seen that night before the anthem finishes and the sky goes back to normal, meaning their faces were not even shown off camera.
(at around 1h 28 mins) Katniss has an hallucination in which Caesar is making a comment about the effects of tracker jacker venom. Caesar made this comment minutes earlier on television, same attire, same words, so this hallucination is presented as a flashback. However Katniss did not have the opportunity to watch such comment.
(at around 1h 18 mins) When the game-makers start the fire to force Katniss back toward the tributes, the video of Katniss that the game-makers are watching, as she is running away from the fire, shows video being shot directly in front of her and following her behind her, but there is no camera-person or drone following her around the area to make this possible. If the cameras in the trees were to catch her running, they wouldn't be shaky and keeping up with her while she runs.
(at around 1h 25 mins) When Caesar Flickerman points out the tracker jacker hive Katniss plans to cut down, Claudius Templesmith says, "Oh... those things are very lethal." Caesar continues immediately with, "Very. For those of you who don't know, tracker jackers are genetically engineered wasps, whose venom causes searing pain, powerful hallucinations, and in extreme cases, death." However, "very lethal" and "in extreme cases [can cause] death" are opposing statements. We see Katniss get stung several times when she cuts down the hive, but when Peeta finds her he tells her run (and she survives with no lasting effects); of the 5 people the hive falls on, only one dies after being unable to escape the swarm. Based on the evidence, it seems both Claudius and Caesar might have spoken incorrectly, with the tracker jackers being able to kill the way a swarm of bees, wasps or ants can kill in extreme cases.
