- A man yells at his sister because all she does is have sex with a guy and doesn't help him manage the apartment.
- A nude girl falls into a bathtub; her bottom and nipples are briefly shown.
- A man and a woman kiss.
- A young couple attempt to have sex twice. The first time they attempt sex, the woman begins to remove her top, but she is fully covered. They are interrupted before anything takes place. The second time they attempt sex, she looks for a place to have sex.

- A man breaks a woman's knee by smashing it with a metal pipe.
- A man is attacked by a ghost in his car, dying offscreen as his screams are heard.
- A young girl is forced to consume the blood of the ghost, in order to trap its spirit inside her and save her sister. She is shown convulsing afterwards.
- A child is violently killed in a room by being thrown across the walls like a rag doll, and his bloody corpse is discovered afterwards, with nearly every bone in his body broken.
- Flashbacks are shown of a man brutally murdering his wife, son, and cat.
- A young woman is dragged into her bathtub by a ghost and drowned. Her body then disappears.
- A woman is attacked by a ghost who emerges from her painting, killing her offscreen. The aftermath is shown later, her corpse with her eyes gouged out and jaw detached with black paint leaking from the wounds, seen again when being brought out on a stretcher.
- A woman's neck is snapped viciously by a ghost, spraying blood onto a door.
- A man shoves another man onto a pile of bricks (some blood is shown).
- A man stabs a woman in the neck with a utility knife and twists it around. When the man pulls the knife out his hand and the knife is shown dripping with blood while the dead woman bleeds on the floor.
- A man is attacked by a ghost in an empty apartment room, with the door slamming shut before we see the ghost kill him. His body is later found in a bathtub, looking deathly pale.
- A ghost chews a hole in a man's throat offscreen, though we hear him screaming as the shot cuts away. His bloody corpse is seen in the next shot, a gaping hole present in his neck.

- The appearance of the ghost of Kayako and her son Toshio can be very frightening and disturbing, and the noises they both make are very sinister and disturbing. They also move and contort unnaturally and in inhuman ways.
- The kills in this movie are bloody and brutal.
- Flashbacks of a man brutally murdering his wife, child, and family pet are shown. This may be disturbing for some viewers.
- The deaths may be disturbing for some viewers.

Frightening & Intense Scenes
- Naoko's ghost, who appears after her brutal death, can be very frightening and intense, as she moves around with stumbles and has blood constantly squirting out of the wound on her neck while making guttural noises, as a result of her knee being smashed and being stabbed in the neck, severing her spine.
