At the very beginning, when Gibbs is at the crime scene, he crosses the 'yellow line' and flashes his NCIS badge at the officer. Midway through the 'flash', the camera shifts to a zoom-out from behind the severed leg. Three seconds later, we see Gibbs take his badge out of his pocket and flash it again.
When DiNozzo is looking up hospital records, "Hospital privileges" is misspelled "priveledges".
Dr. Sylvia Chalmers was recalling the incident where Dorn collapsed in her office and she had called for the ambulance, She said there was only 1 ambulance for the county since it was a small county. However she stated that the ambulance was in Turtle Creek, WV, handling a car wreck. She is incorrect since Turtle Creek, WV is in Boone County and Harmony, WV is in Roane County which is on the opposite side of Kanawha County. This should have made the ambulance for Roane County available. Or they could have called an ambulance from Kanawha County.
Gunnery Sergeant Vestman states that the manual for the LAV-25 'was written by an Army Pogue'. The LAV-25 is a USMC-only vehicle, making it unlikely that the person who wrote the manual would be Army personnel. However, he could be saying that to motivate his men.
Clarksburg, WV is 238 miles from the D.C. Naval Yard; Harmony, WV is another 125 miles southwest from Clarksburg. It is highly unlikely the team would be making multiple trips in the course of a few days.
