In this episode we see a lot of patients making decision that will end their lives. Like the young patient of Doug who is now 18 and wants to be taken of the respiratory device even though he will end up dying. And there is this very well made story about the women with a Down Syndrome who is not put on the heart transplant list. But when Mark finally manages to get her there, turns out that her mother doesn't want it as she doesn't see herself live for too long and doesn't want her to go to a home where she feels life is not of good quality. Both Mark and her mother make great arguments.
