When Frank Conner steals the motorcycle the white helmet is in the helmet lock on the side of the bike yet when he arrives at the hospital it is on his head.
Near the end of the movie when McCabe steals the truck we hear over the radio that the truck he stole was a 89 blue Dodge pick-up. Minutes later we can clearly see a "Ford" logo on the front of the truck.
When Conner is helping a shot officer shot in the arm, the officer's shirt is open showing a stomach wound and his white t-shirt with a big blood stain. In the next shot, the shirt is buttoned and we see his uniform is navy blue. His open shirt is seen in next shot as he is on table being examined.
Near the end of the movie, McCabe is pouring liquid cyclopropane on the floor, from a metal container. As a basic fact of thermodynamics, this action will not only freeze the container valve (thus disabling it), but also the container itself would become so cold that he won't even be able to hold it.
when Peter dislocates his thumb, this would cause an internal rupture and swelling would occur within minutes causing the hand to become larger, at the very least a third of it's normal size, hence defeating the purpose of dislocating his thumb in the first place which is to free himself from restraints.
The liquid cyclopropane, in addition to not being able to be dispensed in the way showed, is no longer used in hospitals. Also, if it were in the hospital for some storage purpose, the containers are bright orange.
McCabe takes Narcan to counteract the anesthetic given to him, except that Narcan is an opiate antagonist. Opiates are given for pain relief, not for anesthesia (propofol is the most common general anesthetic given).
When the surgical team are discussing whether to remove Keaton's restraints, the doctor tells Garcia that the bone marrow sample must be drawn from the iliac crest at the base of the spine. The iliac crests are the highest points on the side of the hips. You can easily feel your own, almost at waist level.
While Frank is leaving the room his son was being treated in after breaking the glass in the door, he pushes on the door frame, causing the door to open slightly. This wouldn't happen if he only smashed the glass.
When Pete was on the computer towards the beginning of the movie you can clearly see that there is only one cord plugged into the computer. So that means that the mouse wasn't plugged in or the keyboard wasn't plugged in, when he was using both.
Rigging to make semi trailer appear to jackknife is visible.
During the car chase, a police unit radios 'I have the blue '89 Ford pickup eastbound on Van Ness'; however, Van Ness Avenue in San Francisco is a principal north-south street.
Despite being present during Peter McCabe's escape and witnessing the deaths of two police officers and one of her nurses, Dr. Hawkins is seen shortly after tending to patients in the emergency room as if nothing had happened. She would have surely been pre-occupied with police questioning in the aftermath of such an ordeal.
